Continuing Care Services are provided to eligible people who need care outside of the hospital, in their home, community or in a long-term care facility. Care can be provided on a long or short-term basis.
Long-Term Care
In Nova Scotia Long Term Care refers to Nursing Homes and Residential Care Facilities. All Long Term Care facilities fall under the jurisdiction of the Homes for Special Care Act and Regulations. The Department of Health and Wellness annually inspects and licenses each facility and approves each facility's budget and per diem rate.
Some facilities provide other services, such as respite beds, adult day programs and outreach services. There are 133 licensed and funded Long Term Care facilities in Nova Scotia. Ownership is mixed with municipally owned, private-non profit, private-for-profit, and hospital-based facilities.
Home Care
Home Care is available to all Nova Scotians who need help with care in their homes and communities. This service is authorized by local Continuing Care offices in the Nova Scotia Health Authority. Home Care Services supplement the help people already receive from their family, friends or community. Through Home Care, people are able to remain as independent as possible by staying in their home and community. It is always the first option for care in the community. The province provides programs and funding that include a variety of supports, from taking care of daily needs like meal preparation to funding for a personal alert system.
There are 15 funded home care agencies in Nova Scotia.
