JOB SUMMARY:

Under the supervision of the manager and with the support of the promotion coordinator or the health promoter for their zone, the successful candidate will work in partnership with local communities. As part of a public health promotion team, they respond to needs that are primarily related to health determinants. With community partners, interdisciplinary teams, and municipalities, they develop strategies designed to address the health needs identified by the community, such as through community health needs assessments (CHNAs), the regional resilience committees' action plans, the regional service commissions' (RSCs) strategic plans, and public health's priority areas for action. The strategies developed aim to increase the community's ability to promote better health for the population in the zone.

The incumbent plays a unifying and facilitating role within the community, through meetings, to stimulate community engagement towards a common goal: meeting the health needs of the population. They build relationships and partnerships throughout their zone, Vitalité Health Network and the province. They work with community members to build a community support network and health-promoting environments.

REQUIREMENTS:

  • Bachelor's degree in a relevant discipline in health or social sciences;
  • CPR certification or willingness to obtain it;
  • Minimum of three years' experience in a community setting demonstrating the skills and abilities listed below;
  • Good knowledge of the demographics and health needs specific to the communities in their zone;
  • Ability to recognize the impact of health determinants;
  • Diligence and ability to use methods that encourage the involvement and participation of individuals and communities;
  • Ability to build and maintain community partnerships;
  • Leadership and good interpersonal skills;
  • Good organizational, prioritization and efficient time and work management skills;
  • Good verbal and written communication skills;
  • Initiative, creativity, self-motivation, positive attitude and flexibility;
  • Valid driver's licence;
  • Ability to work independently and as a team member;
  • Physical ability to perform assigned duties;
  • Good previous work history (performance and attendance);
  • Adherence to professional ethics principles, to the Network management philosophy and organizational values;
  • Compliance with the rules of confidentiality established by the Vitalité Health Network.
