Who you are

Reporting to the Clinical Director of Operations and Patient Flow, with strong collaboration with the Local Chiefs of Staff, the Manager of Patient Flow assumes responsibility for implementing and leading with identified accountability for day-to-day operations at the Saint John Regional Hospital related to Patient Flow Process, Bed Access Coordination and Discharge Planning in collaboration with Community stakeholders. Committed to patient focused care, the Manager of Patient Flow provides leadership and support for a coordinated, interdisciplinary approach to discharge planning creating a culture that supports the right care in the right location by the right provider. This role will involve other projects as determined to be in alignment with the above priority areas. Evaluation and quality improvement activities will be essential for the achieving the successful outcomes required of this role. The local area scope of this role requires the development of strong collaborative relationships with senior administrative leads, Clinical/ Therapeutic/Diagnostic leaders and medical leaders and identified community partners

QUALIFICATIONS:

  • Baccalaureate degree in Nursing
  • Minimum of 10 years of healthcare experience
  • Progressive management leadership (5 years) in leadership, negotiation, and mediation
  • Experience in management, relationship, and partnership building, and delivering results
  • Experience with data analysis and improvement methodologies, as well as proven ability to make data-based decisions to improve the processes
  • Commitment to a patient-focused, action-oriented environment.
  • Organizational and decision-making skills in support of patients, families, and client centred services
  • Ability to develop and maintain collaborative working relationships and partnerships with stakeholders and physicians across sectors
  • Demonstrated ability to evaluate data using outcome measurement, develop and monitor department work plans.
  • Demonstrated ability to create and lead complex work teams
  • Proven ability to work in a fast-paced, rapidly changing environment.
  • Well-developed presentation skills with demonstrated experience in providing effective presentations and leading committees is an asset.
  • Good health and work record.
  • Good working knowledge of MS Office.

Written and spoken competence in English, Written and spoken competence in English is required at an advanced level. bilingual abilities will be considered an asset.

What you will do

KEY RESPONSIBILITIES:

Leading, Planning and Organizing

Responsible for leading the local area patient flow and bed access management. Leads the day-to-day operations for bed access, patient flow coordinators, discharge planners and nursing supervisors involved in patient flow activities.

  • Leads the implementation of identified priorities to improve patient flow and bed access.
  • Active member of the local patient flow committee and ensures that actions are implemented and completed.
  • Liaises with local Health Care Providers to address delays in patient flow due to out of date Expected Dates of Discharge (EDD).
  • Meets with patients and families to identify and remove potential barriers to patient flow
  • Collaborates with clinical directors, managers, and health care teams, to solicit feedback and ideas that will help to inform the development of strategies related to improve patient flow and bed access.
  • Act as a point of contact and collaborates with external partners such as social development, extramural program, Ambulance NB and Vitalité.
  • Creates strong partnerships with both internal and external collaborators to foster a holistic and complete care journey for patients and families. Partnerships include but are not limited to internal collaborators such as clinical directors, physicians, therapeutics and managers along with external collaborators.
  • Lead, develop and create a 'culture' of accountability for all involved in patient flow activities

Program Delivery

Ensures that patients flow activities are delivered in alignment with Horizon Patient Flow strategies

  • Lead a team of professionals to ensure the implementation of initiatives related to patient flow and bed access coordination.
  • Leads their team so they are flexible and able to pivot quickly to address identified priority initiatives or directives.
  • Provides accurate and timely feedback to the Clinical Director of Operations and Patient Flow related to daily metrics and priority issues.
  • Works with emergency management in ensuring the Nursing Supervisors are supported in their duties.

Partnership and Networking

Supports the Clinical Director of Operations and Patient Flow in maintaining collaborative relationships with all partners both internal and external to Horizon in the participation and communication of committees, forums and working group related to key areas of responsibility.

Establish and maintain excellent working relationships with all partners.

  • Develops and implement a strong communication strategy for internal and external stakeholders.
  • Act as a liaison with Department of Social Development and EMP, focusing on improving patient flow experiences while discharging to the long-term care system.
  • Leads discussions with health care team members to support the successful discharge and transition of patients to community
  • Identifies and elevates issues offering potential solutions to remove barriers to discharge
  • Identify and develop partnership with internal and external groups and agencies to continually determine barriers and develop strategies to remove/eliminate barriers to discharge and improve the integration and effectiveness of service delivery
  • Foster collegial relationships and integration within the organization and larger community stakeholders.

Evaluation and Quality

Responsible for providing data to support the evaluation of initiatives and identifies opportunities quality improvements to further advance patient flow and bed access priorities. This will be essential to ensure relevance by strong evaluation processes in a constantly changing environment.

  • Monitor, and report on key performance indicators (KPIs) for all responsible program areas.
  • Ensure the quality of the programs/initiatives and Identify areas for continuous improvement by monitoring and evaluating KPI's, trends and develop and / or supports the development of processes, policy and / or standards related to program responsibilities.
  • Supports, develops, and creates a 'culture' of safety, continuous improvement and creativity aimed at improved patient flow and health outcomes.

NOTE: The above responsibilities are the basic and prime duties; related and allied duties may be assigned from time to time as may become necessary for the operation of the programs.
