MOBILE MENTAL HEALTH & ADDICTIONS CLINIC - SOUTHWEST ONTARIO
Schedule:8-hour shifts Monday-Friday
The Ontario government is connecting people to timely and convenient mental health and addictions services in Huron, Perth, Bruce and Wellington counties by launching a new mobile mental health and addictions clinic in partnership with Medavie Health Services to deliver timely care in the community.
The mobile mental health and addiction clinic will supplement existing local services while creating a new pathway to care. Mobile clinics operate by having a clinical team that travels between communities, reducing the distance that people in rural communities have to go to find care and serving more people in the process.
Several positions are available for interested registered nurses and social workers to support individuals and families living in the counties of Huron, Perth, Bruce and Wellington and surrounding communities. The successful candidates will work collaboratively with other interdisciplinary service providers such as psychologists and other mental health practitioners.
Duties include but are not limited to:
- conduct in-home client visits with Mobile Mental Health Team members;
- provide client support through Tele-Support access line.
- possesses in depth knowledge of suicidal and self-destructive behaviors as well as homicidal and aggressive behaviors;
- accurately assesses client's condition and presenting problem, and contributing factors, taking into consideration the environment, individual, family, and social context;
- conducts mental health and risk assessments to facilitate client rapport; make referral recommendations; and provide mental health and addictions support incorporating client, family and other professionals' information;
- collaborates and shares information with law enforcement and/or Paramedic Services;
- recommends and facilitates referrals to appropriate alternative resources as indicated, including appropriate location for service delivery;
- employs a technical knowledge of psychotropic medication, including expected outcomes and the ability to identify a need for further evaluation or consultation with physician/psychiatry;
- gathers information from interviews with the client as well as collateral information from family and other professionals involved (e.g., physicians, continuing care facilities, supported housing, community partners, caregivers, etc.) where appropriate;
- uses substantive knowledge of numerous areas including but not limited to, mental illness, cognitive functioning, medical co-morbidity, and substance abuse to formulate clinical impressions and recommendations;
- ensures that evidence-based practices are embedded into client care; and other duties as required; and
- other duties as assigned.
Minimum Qualifications:
- considerable clinical experience, preferably involving assessment of and exposure to acute as well as chronic and persistent mental illness; treatment and/or case management of clients with mental illness; work with clients of varied ages and levels of intellect; exposure to addictions related issues, and experience working in varied mental health settings as well as in the community;
- must be registered and in good standing with the Ontario College of Nurses;
- demonstrated ability to provide initial psychological interviews and assessments;
- experience on interdisciplinary teams, in program development and case management;
- requires independent critical thinking, clinical judgment and independent decision-making skills required to develop, implement and monitor care plans;
- must have the ability to work effectively as a reliable member of a team; to work independently with minimal supervision; and to adapt to a changing, evolving environment;
- must have a valid Driver's license in good standing;
- applicants must have a good previous work attendance record;
- the successful applicant must provide a satisfactory criminal record and vulnerable sector check prior to beginning employment.
