About the Role:

Albion Neighbourhood Services is seeking passionate and committed Street Outreach Workers to join our van-based outreach teams. These positions play a vital role in engaging with individuals experiencing homelessness across our designated catchment areas within Ward 1,2,3,4, 5, 7,8 and 9.
The goal is to connect clients with housing supports, emergency shelters, health services, and other community resources, while fostering pathways to long-term housing stability and self-sufficiency. The day and evening shift will work collaboratively to assist clients in accessing services that can help them to achieve greater self-sufficiency and resiliency.

This position will be reporting to the Street Outreach Manager.

Full-Time Day Shift (MondayFriday, 8:30 AM4:00 PM) 2 Positions available

Full-Time Evening Shift (MondayFriday, 3:00 PM10:30 PM) 3 Positions available

Mandatory Job Conditions:
Maintaining a clean driving record is mandatory requirement and will be periodically verified through the agencys insurance provider.
A Vulnerable Sector Police Check is required and subject to annual re-screening.
Outreach work requires navigating various environments, including outdoor encampments, shelters, and public spaces across seasons. Employees are required to wear protective gears.

Key Responsibilities:
Conduct van-based and foot outreach & housing supports by engaging with homeless individuals in parks, bus shelters, and other areas, offering support, wellness check ins, referrals, assistance with shelters and other services available during day or evening hours.
Provide timely responses to referrals, including Concerned Citizen Calls and 311 routed through the City of Toronto.
Assist clients with securing and maintaining housing, including housing searches, landlord mediation, ID replacement, and barrier resolution.
Support clients in completing and maintaining applications for housing (e.g., Rent Caf, supportive housing), as well as housing stabilization grants (HSF, Bridging Grant, Housing Allowance/COHB, PATHS).
Provide transportation for clients to appointments, shelters, drop-ins, or other safe locations in the agency van.
Provide harm reduction services, basic needs supports to the homeless clients during outreach.
Maintain the agency vehicle, ensuring safety and functionality, and report concerns to the Team Leader.
Deliver enhanced outreach during extreme weather alerts (cold/heat).
Provide supportive counseling, case management, referrals, and harm reduction services (including naloxone distribution/administration).
Maintain accurate, up-to-date client records in accordance with funder and agency requirements (logbooks, SMIS, TGRIP, nFocus etc.).
Engage with community stakeholders, including Streets to Homes, City of Toronto, local businesses, and other service providers.
Attend team meetings, funder meetings, and participate in ongoing training relevant to the role.
Complete required reports (daily, monthly, semi-annual, year-end) as per City of Toronto and agency guidelines.
Other duties as assigned, in alignment with funder requirements.

Qualifications and Requirements:
Post-secondary degree or diploma in Social Work or related field, or equivalent combination of education and experience.
Minimum 3 years of experience working directly with individuals experiencing homelessness or vulnerable population preferably in a street outreach setting.
Strong oral and written communication skills, with the ability to enagage, build trust and rapport with marginalized populations.
Demonstrated knowledge of homelessness, mental health, substance use, harm reduction, and community resources.
Strong problem-solving, crisis intervention, and counseling skills.
Valid Class G Drivers License and clean driving record. A clean driving abstract must be presented at the time of interview.
Must be comfortable driving an outreach van in all weather conditions.
A Negative Vulnerable Sector Police Check (required prior to employment and subject to periodic review).
Flexibility to work on statutory holidays and other agency closures.

Position start date: May 2025

Compensation: $31.94 per hour (includes pay equity)
