Job Summary:
As the primary point of contact for individuals and families seeking support, information, and guidance related to dementia, the First Link Clinic Coordinator plays a vital role in building trusting relationships and providing person-centred support. Working within a healthcare setting, the First Link Clinic Coordinator collaborates with healthcare providers, clinic staff, community organizations, and support services to connect clients with appro-priate resources and ensure they receive timely, compassionate, and coordinated support throughout their de-mentia journey.
This is a full-time, on-site position based within a clinical setting, and candidates must be available to work in person during regular clinic hours.
Distinguishing Characteristics:
The ideal candidate is compassionate, organized, and skilled in client support, with the ability to navigate sensi-tive conversations and complex situations. This position serves as a key liaison between individuals living with dementia, their care partners, healthcare professionals, and community resources. The successful candidate will be committed to providing exceptional client service while helping individuals and families access the infor-mation, programs, and supports they need to maintain quality of life.
Typical Duties and Responsibilities:
- Provide person-centred support, education, and navigation services to individuals living with Alzheimer's disease and other dementias, as well as their care partners.
- Receive and respond to referrals from healthcare providers and clinic staff in a timely and professional manner.
- Conduct needs assessments to identify client strengths, challenges, and support requirements.
- Connect clients and families with appropriate community resources, support services, educational op-portunities, and dementia-specific programs.
- Maintain ongoing communication and follow-up with clients and families to ensure changing needs are identified and addressed.
- Build and maintain collaborative relationships with physicians, nurse practitioners, allied health profes-sionals, clinic staff, and community partners.
- Maintain accurate, confidential, and up-to-date client records in accordance with privacy legislation and organizational policies.
- Provide information packages, educational resources, and toolkits to clients and families.
- Participate in case consultations, care planning discussions, and interdisciplinary meetings when appro-priate.
- Deliver presentations and educational sessions to healthcare providers, community organizations, and the public as required.
- Support awareness initiatives that promote understanding of dementia and available supports within the community.
- Maintain the CRM database and ensure timely and accurate data entry for reporting purposes.
- Participate in provincial and national initiatives, meetings, and committees as requested.
- Travel within the region as required.
- Perform other duties related to client service delivery and organizational priorities as assigned.
Qualifications:
- Post-secondary education in a related field, or an equivalent combination of education, training, and relevant experience.
- Experience working in healthcare, community services, social services, client support, administration, or a related environment.
- Experience working within a healthcare or clinical setting is considered an asset.
- Ability to build positive and collaborative relationships with healthcare professionals, clients, families, and community partners.
- Demonstrated understanding of the impact of Alzheimer's disease and other dementias on individuals, families, and care partners.
- Strong interpersonal, communication, and active listening skills, with the ability to navigate sensitive and emotionally challenging conversations.
- Demonstrated ability to maintain confidentiality and handle sensitive information with professionalism and discretion.
- Strong organizational, time management, problem-solving, and administrative skills, with the ability to manage multiple priorities effectively.
- Ability to work independently while contributing positively as part of a team.
- Proficiency in Microsoft Office applications and experience using databases, client information systems, or CRM platforms.
- Bilingualism (English and French) is considered a strong asset.
- Possession of a valid New Brunswick driver's licence and access to reliable transportation.
- Ability to travel within the region and work occasional evenings or weekends as required.
- The successful candidate must provide and maintain a satisfactory Criminal Record Check and Vulnera-ble Sector Check as a condition of employment.

Annual Salary: $48,000.00 per year
