Definition of work:This is an advanced professional level position involving a high degree of responsibility, autonomy and independence in identifying and providing specialized assessment, individual service planning and critical intervention services.
The successful candidate will ensure that quality clinical intervention is of a consistently high standard, and in keeping with Regional and provincial home support policies and operational standards. Duties include, clinical case consultation; assessment of self and staff learning needs; identification of educational opportunities; analysis of workload measurement; policy/program development and evaluation; research; trend analysis; liaising with both internal and external partners and other professionals; review / processing of home support requests; professional reporting, auditing of home support agencies and documentation standards; responding to inquiries, compliments, complaints and other related duties. Will be responsible to review, panel, approve and coordinate placement into Personal Care Homes. Will be responsible as the coordinator of the monitoring team attached to personal care homes. Will be responsible to gather information and complete reports and reviews of personal care home in accordance with the Personal Care home Provincial Operations Standards.
The coordinator will facilitate staff orientation; ensure compliance with professional standards; develop and implement teaching and self-directed learning modules to support the team’s clinical practice; provide guidance and consultation to professional staff in the area of home support services and; adhere to standards for client services in accordance with the Canadian Association of Social Workers Code of Ethics, the Standards of Practice in Social Work, and the Social Workers Act of Newfoundland and Labrador. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
• Bachelor Degree in Social Work.
• Current registration with NLASW.
• Minimum of three years current experience (within the last five years) in home support services.
• Demonstrated extensive knowledge in coordinating programs for seniors and persons with disabilities; Home Support program and related benefits; the adult needs assessment/reassessment process; and client care assessment, planning and coordination.
•Will need experience with RAI assessment tool and CRMS as well as Personal Care Home Program and placement services.
• Demonstrated ability to establish and maintain effective working relationships with others including clients, families, colleagues and relevant outside agencies.
• Established analytical capability and oral / written communication skills.
• Ability to work both independently and as part of an interdisciplinary team.
• Demonstrated ability to develop, plan, implement and facilitate orientation as well as other training/education sessions with a commitment to ongoing professional development for self and others.
• Demonstrated effective conflict resolution and leadership skills.
• Demonstrated proficiency in the use of computers.
• Availability to participate in an on-call rotation.
• Use of a private vehicle is required.
