BRIEF DESCRIPTION
The Guelph-Wellington HART Hub offerslow-barrier health care, mental health and substance use supports for people (16+) who are experiencing or at-risk of homelessness, with complex health conditions, including moderate to severe mental health and substance use challenges.As part of the HART Hub, Guelph CHC runsan Intensive Housing and Treatment Team that offers onsite and outreach services, including primary care, episodic health care, support coordination, counselling, access basic needs, treatment, and recovery supports.
Guelph CHC's HART Hub team will work very closely with other HART Hub programs and community-based organizations supporting the same priority population, including withdrawal management services, crisis stabilization, permanent supportive housing,sheltersand outreach teams. As part of a broader community vision to create a continuum of care, Guelph CHC's HART hub team will provide health care, support, connect, refer and engage in shared care planning for HART Hub clients, with the goal of providing wrap-around, stage-matched supports for clients.
PRINCIPAL JOB DUTIES
- Identifyclient-centred goals that lead to outcomes that are meaningful and relevant to the individual
- Use a trauma-informedapproachtodeliver stage matched interventions
- Collaborate with other provider types within the Hub team to provide wraparound care
- Support clients in navigating socialassistance, housing, food security,transportationand other needs as pertains to their well-being
- Support thedevelopment andimplementation ofcare plans that address clients health, basic needs, mental health substance use,recoveryandtreatmentneeds
- Maintains client records, assessmentnotesand correspondence within the safeguard of confidentiality
- Provide accompaniment and advocacy to ensure clients can access the care they need
- Liaise with other care providers in the system(e.g. Hospital, primary care, outreach services)tosupport coordinatedcare forclients
- Participate in community groups and committees relating to the development and delivery of health care and social services in the community
KNOWLEDGE, SKILLS & ABILITIES
- Post Secondary degree or diploma in a social service discipline
- Emergency First Aid &CPRLevel CCertification Mandatory.
- Drivers license and insurance Mandatory
- Experience working withindividuals marginalized by theirmental and/orsubstanceusehealth, alienated fromservices, andfrequentlyincrisis, both in-patient and community settings.
- Demonstrated knowledge of community resources related to the served population in Guelph.
- Ability to work independently and within an interdisciplinaryteam environment.
- Working knowledge of cognitive behavioral therapy and dialectical behavioraltherapyan asset.
- Ability to advocate effectivelyon behalf of clients to improve access to resources and support clients in meeting their goals.
- Proficient in Microsoft Office.
- Engaging, collaborative and creative,astute toclients' needs with the ability to adapt and be flexible.
- Excellent assessment,communicationand organizational skills.
WORKING ENVIRONMENT
- Some evening workrequired
- 60% client/ 1:1 interaction, 40% desk, meetings, frequent change of position
- Exposed to stress due to dealing with clients in difficult personal circumstances.
