Job Description: The Senior Social Worker provides support, advocacy, and discharge planning within acute care with the Discharge Planning and Support Team (DPAS) reporting directly to the Manager. The Senior Social Worker is an experienced clinician who provides short-term case management support to patients in acute care and urgent care areas . The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to assess and support patients and/or to coach, mentor, orientate and provide education to an assigned team in DPAS. Drawing on standards of practice for case management; professional codes of ethics; social work standards of practice in health care; program policy and procedures; and relevant legislation, the Senior Social Worker provides high quality patient service. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

  • Three years recognized clinical experience with demonstrated leadership ability.
