Job Description: The Patient Care Coordinator follows assigned clients from admission to discharge providing case management through assessment, coordination, implementation, monitoring and evaluation of health-related services to clients. Quality care and cost-effective outcomes are promoted with the client's expressed health goals and general well-being at the forefront of the care plan. The Patient Care Coordinator maintains a collaborative client-centered approach that is continuous across provider and agency. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education

  • Bachelor of Social Work (BSW)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Bachelor of Nursing is acceptable
