The Department of Health is seeking an individual to join the Primary Health Care Branch as the Collaborative Care Clinic Implementation Manager. The successful candidate will report to the Director of Primary Health Care Strategy & Innovation.

As the Collaborative Care Clinic Implementation Manager, you will:

  • Lead stakeholder engagement with family physicians, nurse practitioners, community partners, the Regional Health Authorities, etc.;
  • Provide information on funding models, scope of practice, regulatory requirements, and clinical governance structure;
  • Conduct readiness assessments to help practices identify opportunities, benefits, risks, and capacity needs for joining collaborative care clinics;
  • Support providers in developing implementation plans based on local context, provider readiness, and resources available;
  • Manage contracts/agreements with physicians, nurse practitioners, clinics, etc.;
  • Complete monthly reviews of indicators, targets, and data to monitor clinic performance, operational stability, and adherence to standards;
  • Serve as the liaison to support continuous improvement efforts, contract compliance, workflow challenges, sustainability risks, and support action plans to optimize clinic goals;
  • Facilitate coordinated support from Practice Support and Practice Facilitation teams to strengthen workflows, improve efficiency, and ensure a continuous improvement mindset;
  • Ensure clinics have access to tools, templates, and monthly reports to support care delivery;
  • Provide support to clinics to help manage change processes in clinic environments with minimal standardized structure;
  • Provide supervision, coaching, and performance management to staff on the team;
  • Monitor staff workload, and capacity, adjusting staff assignments as needed to meet team and project demands;
  • Contribute to Branch strategic planning, policy development, priority setting, and evaluation of initiatives;
  • Perform various other duties as required.

Essential Qualifications:

  • Bachelor's in Business Administration, Engineering, or another health-related field, and
  • Five (5) years of relevant experience.
  • An equivalent combination of education, training and experience may be considered.
  • Written and spoken competence in English and French is required. Please state your language capability on your application.

In the event that this competition poses difficulties in recruiting a qualified candidate, a covering off appointment at a lower level may be made. In that case, candidates with less than the required related years of experience may also be considered.

Asset Qualifications: Preference may be given to candidates that demonstrate the following asset qualifications:

  • Experience with, or knowledge of, Project Management methodologies and principles.
  • Experience with, or knowledge of, Change Management principles.
  • Experience managing employees.
  • Experience with, or knowledge of, the health care system.

Salary: $2,991 to $3,979 bi-weekly (Pay Band 6)
