Nursing Home Without Walls Assistant Coordinator Position

DREW NURSING HOME

An opening is available for a full-time Assistant Coordinator position with the Nursing Home Without Walls (NHWW) program. This position is for a one-year term, with potential to continue.

The successful candidate will assist in providing supportive services, under the guidance and management of the Nursing Home Without Walls Program Coordinator, at the Drew Nursing Home.

Role and Responsibilities

Support the continued implementation of the Nursing Home Without Walls community outreach program for older adults who are 60 years of age and older in Tantramar, New Brunswick. Assist the program Coordinator with administrative duties and providing numerous social health initiatives that empower individuals' goals to age in place, while reducing loneliness and isolation.

Work within the principles of:

· Respecting the contributions of older adults or seniors, their families, caregivers, as well as having awareness of the assets within the community.

· Nurturing meaningful participation, collaboration, and partnerships.

· Encouraging health through holistic wellness, community connection, and lifelong learning.

· Maintaining independence, choices, and dignity for older adults.

· Upholding a commitment to and respect for diversity and equity.

· Supporting person-centered and community-based programming.

Specific accountabilities:

· Answer and return phone calls; maintain organized files and records.

· Document and communicate pertinent information to Coordinator and clients.

· Execute surveys and enter relevant client data, as needed.

· Perform consistent check-in phone calls, as well as friendly home visits.

· Provide safe, reliable transportation to medical appointments, NHWW social activities, and errands days, as required.

· Uphold client confidentiality and positive, professional interpersonal relationships.

· Assist Coordinator with navigating a variety of systems, including health care and the Department of Social Development services and applications, as well as other applicable federal, provincial, and local resources.

· Support the continued facilitation of NHWW social activities, gatherings, educational opportunities, and established programming; opportunity to create additional services and activities, based on Assistant Coordinator strengths and community members' feedback.

Additional Notes

Business hours are typically 8:30 a.m. to 4:30 p.m.; however, occasional weekends and evenings may be required.

This position includes working collaboratively within a team, as well as under the direction of the Nursing Home Without Walls program Coordinator and Drew Nursing Home Executive Director.

Qualifications and Education Requirements

University or college education in areas of health care, administration, geriatrics, sociology, social determinants of health, non-profit organizations, asset-based community development, and/or health and well-being studies related to the position.

Excellent written and verbal communication.

Knowledge of Nursing Home Without Walls programs, services, and goals; ability to effectively communicate information about services and resources.

Understanding of aging, age-friendly community goals, and aging in place objectives.

Ability to learn and navigate information, resources, applications, and supportive services with and on behalf of Nursing Home Without Walls clients, as required.
