POSITION VACANCY

POSITION: Resident Care Navigator

LOCATION: Yarmouth,NS

TYPE: Full-Time

Villa Saint-Joseph du Lac, located in Yarmouth NS, is home to 79 long-term care Residents. We are a provincially funded, non-profit organization that provides 24/7 care. Villa Saint-Joseph has a strong interprofessional team and a new facility (opened in 2015) which allows us to promote a safe, clean, home-like environment for our Resident and care partners.

Responsibilities (including but not limited to):

  • Be responsible for ensuring high-quality support for our residents while fostering a positive and collaborative environment for families, staff, and external partners.
  • Facilitate regular Family Council meetings and participating in Resident Council meetings to promote open communication and engagement between families, residents, and the facility.
  • Advocate for residents' well-being and rights, including support for financial and legal issues
  • Support the admissions process for residents and families.
  • Serve as a resource for residents and their families as well as the interdisciplinary team
  • Actively participate in quality improvement initiatives, including facility surveys to identify areas for improvement, and serve as a liaison with external organizations to connect residents and families with necessary resources and support.
  • Communicate and support residents on social and emotional concerns
  • Collaborate with other professionals to evaluate residents' mental health, medical or physical condition and assess residents' needs

Qualifications:

  • Currently Licensed with the Nova Scotia College of Social Workers (NSCSW)
  • Strong leadership, exceptional communication skills and a deep understanding of resident-centered care practices
  • Effective communication, problem resolution, and teamwork are essential,
  • Past Long Term Care experience is an asset
  • Must have excellent conflict management skills
  • May require flexibility with work hours at times
