Work schedule may be changed to meet operational needs.
Conduct follow-ups with patients in relation to the integrated nutrition pathway for acute care (INPAC). Develop therapeutic menus based on individual care plans (e.g. kidney patterns, carb count, diabetic pattern) and modify them to ensure an appropriate choice to meet the patient's needs. Ensure that each menu corresponds to the prescribed diet and the allergies and intolerances of the patient. Meet with some patients to obtain the information needed to implement the nutritional care plan. Also collect information from family members, dietitians and nursing staff. Work in collaboration with the dietitian, staff from the nursing units and clinical nutrition. Ensure clinical documentation in the patient's file of the interventions implemented. Perform any other related duties according to departmental needs. Follow the department policies and procedures.
  • High school diploma or general education diploma (GED);
  • Successful completion of a recognized food services management and clinical nutrition course of up to six months in duration;
  • Computer skills (Meditech, Microsoft Office Suite);
  • Experience relevant to the position;
  • Demonstrated ability to perform assigned tasks quickly;
  • Proven ability to organize and manage own work;
  • Ability to work independently and as a team member;
  • Physical ability to perform assigned duties;
  • Good previous work history (performance and attendance);
  • Adherence to professional ethics principles, to the Network management philosophy and organizational values;
  • Compliance with the rules of confidentiality established by the Vitalité Health Network.

Written and spoken competence in French and English are required.
