We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.

The Intensive Case Manager is reporting to the Manager of Halton Housing Services, this position will assist in providing intake support and carrying out day-to-day administrative services. Wesley Halton Housing offers services to the homeless in the Halton Region, supporting homeless families and individuals in a transitional program on-site. Services are based on a client-centred, residential harm reduction, housing-first approach to service delivery.

This position is a unionized, full-time, temporary position. The contract end date is October 28, 2024. The rate of pay is $23.67 per hour. The hours for this position are 40 per week.

Key Responsibilities:

  1. Works with assigned families and individuals in a housing program which works towards a housing plan. This includes maximizing income, filing taxes, obtaining ID, and connecting with community supports.
  2. Responds to calls and referrals from community agencies in accordance with caseload
  3. Places individuals and families into permanent leased units, securing appropriate housing based on individual needs.
  4. Assesses and screens participant's/families' needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources.
  5. Completes Social & Community Housing applications (HATCH) to submit to the Region, including all required documents for approval OR ensures existing application remains active
  6. Assess clients for any recent, current, or ongoing experience with domestic violence and make referrals to appropriate programs
  7. Assist participants with their move into independent accommodations and provide neighborhood orientation.
  8. Maintain regular, effective communication with social housing providers, administrators, community partners, and private landlords, to ensure an integrated approach that will provide the best support possible for clients to maintain housing.
  9. Report to the appropriate child welfare agency (CAS) if child abuse and/or neglect is suspected or witnessed.
  10. Conduct regular visits with participants in their homes or shelters to provide support specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building, grocery shopping, cleaning, etc.
  11. Maintain appropriate and accurate record-keeping practice, as per expectations of Wesley
  12. Supports approximately 1 shift per month of 24-hour intake; flex time is available for additional hours worked.
  13. Other duties as assigned.

What you will bring:

  1. Post-secondary degree/diploma in Human Services or related field
  2. Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
  3. Experience working with adults and families with mental health and addictions issues and the homeless and providing non-judgmental support to them.
  4. Must be able to work flexible shifts including day, evening, and weekend hours as well as 24-hour intake responsibilities
  5. Must be able to meet travel expectations of the position; a valid Driver's license and access to a vehicle is a definite asset
