OVERVIEW: Reporting to the Northern Care Program Manager and Supervisor, the Community Coordinator is responsible for coordinating and monitoring community-based placement services including liaising with collaterals and working with House Parents, Respite, and Support Workers

KEY RESPONSIBILITES:

  • Building and maintaining relationships with the Support and Respite Workers, House Parents, Community Resources, Southeast Child and Family Services, and community.
  • Work in partnership with Support Work Coordinator to ensure respite and support worker roles are filled and scheduled appropriately. Managing conflict within the home.
  • Maintaining contact with Airlines and completing pick up and drop offs from airport/dock.
  • Working closely with assigned Case Managers for the home to complete check ins and ensure the home is following safety and case plans. Completing paperwork such as Place's of Safety or Foster Care Applications with House Parents.
  • Picking up, hauling, and transporting freight for the homes and office.
  • Keeping office clean and tidy, and communicating with Visiting Home Coordinator to prepare for internal or outside collateral accommodation.
  • Recruitment of Respite & Support Workers within the community.
  • Picking up and transporting children in the homes for family visits.
  • Supervise overall activities of House Parents, ensuring licensing standards are being followed.
  • Transporting to the store and Nursing Station.
  • Providing supervision to the children when needed in the home.
  • Completing Weekly Home Checklists and delivering paperwork (Timesheets, Monthly Reports, Logs).
  • Deliver an efficient, community-asset based approach to providing care.
  • Participation in community events.
  • Conduct unannounced home visits at supervisor request.
  • Complete documentation of home visits and provide to NCP Supervisor as requested.
  • Receive, help respite, and support workers fill out, review, and submit time sheets and file recordings to Support Worker Coordinator.
  • Follow up with concerns or incidents that take place in the home with direction.
  • Participate in an on-call rotation for assistance after hours.
  • Other duties as required.

QUALIFICATIONS:

  • Grade 12 education.
  • Ability to effectively manage conflict.
  • Knowledge of Community-based resources.
  • Knowledge of the Child and Family Services (CFS) sector and legislation.
  • Demonstrate knowledge and/or understanding of Indigenous perspectives, First Nations culture and traditional philosophies.
  • Strong interpersonal skills and ability to work in a community-based environment with a high attention to detail.
  • Strong proficiency in Microsoft Office, Word, Excel, Outlook and Internet.
  • Excellent oral and written communication skills and ability to communicate in Ojibway.
  • Must have a valid Manitoba driver's license and a reliable vehicle.
  • Must have a current satisfactory Child Abuse Registry Check, Criminal Record Check (including Vulnerable Sector Search), and Prior Contact Check as a condition of employment.
