Position Summary:
The Community and Inclusion Support Worker plays a key role in supporting individuals with developmental disabilities to participate fully in their communities. Guided by the values of the Passport Program and Developmental Services Ontario (DSO), this position focuses on empowering individuals to achieve their goals through skill-building, social connection, and access to inclusive community experiences. Support is person-directed, respectful, and responsive to the unique strengths, needs, and aspirations of each individual.
Key Responsibilities:
- Support individuals in identifying and pursuing personal goals, interests, and community-based activities that promote inclusion and well-being.
- Facilitate participation in recreation, education, volunteering, employment, and social engagement within the community.
- Assist with life and social skills development, such as communication, personal organization, transit training, and safety awareness.
- Promote and model inclusive practices that foster independence, dignity, and meaningful relationships.
- Provide respectful personal support where needed, including physical assistance, emotional support, or help with daily living activities.
- Maintain accurate documentation and records in accordance with Philip & Co. and Passport Program accountability guidelines.
- Communicate regularly with the person supported, their families, and the support coordination team.
- Uphold the rights, privacy, and safety of the individuals supported at all times.
- Attend training and team meetings as required.
Qualifications:
- Previous experience working with individuals with developmental disabilities in a community setting.
- Strong understanding of the principles of person-directed support and community inclusion.
- Familiarity with the Passport Program and Developmental Services Ontario (DSO) framework.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently, with flexibility and reliability.
- Valid Ontario driver's license and access to a reliable vehicle (travel throughout Northumberland County required), with $2,000,000.00 liability insurance.
- Current Vulnerable Sector Check within 30 days of hire.
- Standard First Aid and CPR certification (or willingness to obtain).

Working Conditions:
Work is performed in community settings (homes, public venues, workplaces, etc.) across Northumberland County.
Flexible hours are required, including occasional evenings or weekends, based on individual needs.
