We are seeking a dynamic, highly motivated Community Housing Support Worker who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.
The Community Housing Support Worker is a mobile position providing eviction prevention services to participants and their families residing in community housing sites. This position supports clients, to reduce evictions and tenants need to access emergency shelter, and works with tenants and property managers to problem solve when tenancies are at risk. The CHSW position is 100% mobile; the worker must be in the community connecting with tenants, property managers, and Region of Halton staff. The CHSW is required to travel across the Region of Halton, including Halton Hills, Milton, Oakville, and Burlington. This includes working on-site in community housing buildings to engage clients accordingly.
This position is a unionized, full-time, permanent position. The rate of pay is $26.88 per hour. Hours per week are 40.
Key Responsibilities:
- Identifies, engages and connects with individuals or families who are at risk of eviction from Community Housing sites or are experiencing challenging life circumstances.
- Work collaboratively with other key stakeholders in particular Halton Region to provide an integrated service that compliments and builds upon other services
- Assess participants needs for services and level of intervention to optimize their capacity using needs assessments, individualized action plans and one-to-one support
- Connect and anchor tenants with their communities through creating effective pathways to make sustainable connections with services and activities that will support them remaining in their homes
- Facilitate training opportunities for Community Housing providers to increase their internal capacity, such as Mental Health First Aid, LTB rights & responsibilities, etc.
- Develop and maintain effective communication systems with internal and external staff, clients, and community partners in particular other programs staff working with Community Housing & Halton Region
- Facilitates case conference meetings, participates in the co-ordination of appropriate supports required by participant to ensure an adequate wrap around approach of services has been established.
- Conducts regular visits with participants in their homes to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building life skills, grocery shopping, cleaning, etc.
- Provides supports to participants in regards to their rights under the Residential Tenancy Act.
- Collect data to evaluate and analyze results in order to plan for future sustainable programming, and complete administrative tasks, maintains effective recording/tracking systems and complete documentation in a timely manner.
- Adhere to the policies and procedures of Wesley.
What you will bring:
- Bachelors education in the field of Gerontology, Social Work, or other discipline related to the specific needs of older adults, a Valid Drivers License with access to a vehicle, Non-violent Crisis Intervention, and First Aid/CPR training is required
- At least two years experience in social services, in particular with seniors
- Excellent problem-solving, organization, and coordination skills, and ability to work effectively independently by implementing self-accountability techniques and a strong understanding of support services in the community, specifically those related to seniors.
- Proven ability to demonstrate Wesleys mission, vision, and values.
- Solid communication skills, both written and verbal in English. Second language relevant to the client population is an asset.
