Program Overview – Home & Cluster Care, Community Care
As a leader in the Community Care Unit the incumbent is accountable to the Director, Seniors Wellness for strengthening professional practice of Managers, Supervisors, Personal Support Workers and support care providers. The incumbent is responsible for providing advice, teaching, resources and promoting standards and best practice in Supervisory, personal support care, infection prevention and control, medication management, documentation and coordinated care planning. The incumbent also acts as an expert resource to the interprofessional team in teaching and implementation of controlled, non-controlled acts and assigned tasks and to clients in teaching and learning related to senior's health issues.
The Manager, Professional Practice will collaborate with the Assisted Living Services and Community Programs to provide clients with the objective of enhancing their quality of life through the promotion of health and wellness. The incumbent is responsible for ensuring that the activities and services are delivered in a caring and flexible manner, in keeping with the organization's mission, goals and values.
What You Will Do
  • Provides teaching, advice and support to Mangers, Supervisors, Personal Support Workers, Activations, placement students, and volunteers to achieve operational goals and targets in the Community Care Unit.
  • Facilitates regular workshops on senior health issues.
  • Understands and acts as an expert resource related to College of Nurses of Ontario and Ontario Personal Support Worker Association practice standards and guidelines.
  • Understands and acts as an expert resource related to the scope of practice/controlled and non-controlled acts model as documented in the Nursing Act, 1991, and the Regulated Health Professions Act, 1991 (RHPA).
  • Applies nursing theory to practice via the use of critical thinking and problem-solving skills consistent with RPN or RN educational preparation.
  • Provides safe, competent and ethical nursing care.
  • Ensures program expenditures are consistent with budgetary allocations and in accordance with WoodGreen and funder restrictions, in collaboration with the Manager.
  • Facilitates the coordinated care planning process for those complex clients who would benefit from a nurse leading and facilitating the process.
  • Provides strategic advice to the Director on the best use of available space for brokering health care services to meet client needs.
  • Responsible for the collection and submission of statistical information.
  • Ensures that the health and safety of the clients, volunteers and staff are met; responds to emergency situations when necessary and completes necessary reports.
  • Participates in the annual program review and planning process in Community Care.
  • Ensures the anti-racism objectives and community development principles of the organization are achieved within the programs.
  • Performs other related duties as required from time to time.

What You Bring to the Team
  • Registered Practical Nurse Diploma OR Registered Nursing Degree (BScN); valid certificate of registration with the College of Nurses (maintained annually)
  • Master or Health or Business Administration or related (an asset)
  • 5 years of significant direct experience and acute clinical-related experience working with older adults and frail and vulnerable seniors.
  • Strong understanding and knowledge on seniors' health and wellness issues.
  • Expert understanding of health promotion and chronic disease management related to aging.

  • Strong ability to multi-task, with the capacity to prioritize and balance competing demands
  • Outstanding communication skills with the ability to convey complex issues and influence others.
  • Strong interpersonal skills with the ability to manage tense or emotional charged situations with tact and diplomacy and mediate conflict between others.
  • Knowledge of the Regulated Health Professions Act 1991 (RHPA).
  • Knowledge of College of Nurses of Ontario and Ontario Personal Support Workers Association practice standards and guidelines.
  • Knowledge of Infection Prevention and Control (IPAC) measures for Health Care Workers.
  • Second language is an asset.
  • Capable of fulfilling the physical and mental demands of the job as per the Position Demands Analysis.
