The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access safety, basic needs, health care and other mental health and addiction services under one location.
The Nurse Practitioner (NP) within the Hubs program, will perform duties and responsibilities consistent with providing clients integrated mental health and addiction-based medicine and primary care. The NP provides direct and indirect comprehensive patient care services to support patient care in collaboration with various inter-professional teams across the organization in order to support mental health and addictions needs, including provision of Withdrawal Management support.

- What you'll be making - Starting rate for this role is $65.69 per hour (Level 1 of the Nurse Practitioner Grid);
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $70.87 per hour
- Internal applicants will be placed on the grid based on their seniority
- When you'll work - 70 hours over two-week period; Monday-Friday 8am-4pm

HOW YOU WILL BE MAKING A DIFFERENCE:
- Provide Services addressing both addictions-related and general medical needs
- Provide non-urgent, health care services within the NP's full scope of practice to clients who encounter barriers to accessing primary health care. Consults with collaborative physicians for clients who have unstable/complex health conditions
- Coordination with Psychiatrists and other team members for comprehensive care.
- Maintain requirements for practice, including a license for a Registered Nurse in the Extended Class, NP/Physician Collaborative Practice Agreement, and malpractice insurance
- Maintain confidentiality and act in an ethical manner, consistent with professional expectations and the College of Nurses of Ontario (CNO)
- Retain clinical competence through relevant evidence-based research and legislation
- Excellent communication, decision-making, problem-solving, conflict management and time management skills
- Demonstrate ability to work effectively in an interdisciplinary team environment
- Perform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance to the Practice Standard: Nurse Practitioner (2018)
- Determine the need for follow-up appointments and assist with transfer of care to permanent primary health care services
- Provide support for additional HUB sites as required
- Demonstrates an understanding of the neurobiology of addiction
- Advanced practice assessments and interventions for a diverse range of clients with addiction problems
- Assesses and manages patients with addictions who have co-existing health conditions including but not limited to mental health conditions, blood borne diseases and infections
- Advanced health assessment to identify clients' emotional, cognitive and behavioural states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal
- Utilizes evidence‐based interventions, including withdrawal and other medication management, to meet the needs of a diverse range of clients with addiction problems
- Provides care to persons experiencing an addiction and / or mental health condition that is recovery oriented, trauma-informed, and addresses social determinants of health
- Engages clients in strengths-based care that promotes resilience
- Provides education to individuals and families about addiction and its management
- Facilitates and engages in collaborative, inter- and intra-professional, and intersectoral practice when providing care for persons with an addiction(s)
- Understands stigma as a barrier and assists and supports patients and families
- Demonstrates knowledge related to the process of voluntary and involuntary care
- Acts as a resource providing addiction‐related clinical advice for health and other professionals, community groups and clients and their families
- Advocates for persons experiencing an addiction
- Provide health care services to clients 16 years and older that meet eligibility
- Provide leadership and guidance for the development of expanded medical services within the agency
- Advanced practice assessments and interventions for a diverse range of clients with addiction problems
- Assesses and manages patients with addictions who have co-existing health conditions including but not limited to mental health conditions, blood borne diseases and infections
- Advanced health assessment to identify clients' emotional, cognitive and behavioural states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal

- Identifies and seeks new knowledge, skills, and supports related to care of patients with addictions and/or mental health conditions
- Demonstrate ability to deal effectively with sensitive issues with clients and families using a wellness production and client-centred approach
- Determine the need for follow-up appointments and assist with transfer of care to permanent primary health care services
- Work in partnership with the Withdrawal Management Centre and other community agencies and programs
- Integrate organization counselling services and support for clients
- Coordinate service provision with other health care partners in the development of community care plans and wrap-around services
- Assist in program development to align with best practice standards, evidence-based research and organization program mandate
- Promote the integration of addiction-based medicine and primary care
- Offer educational opportunities and consultation as appropriate
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
- Current registration with the College of Nurses of Ontario as a Registered Nurse (in the Extended Class) (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Successful completion of the Prescribing Narcotics and Controlled Substances course
- Current certificate in cardiopulmonary resuscitation (CPR)
- Valid Driver's License, adequate insurance and access to a vehicle to carry out job duties
- Current, clear Police Vulnerable Sector Check (PVSC)
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- Minimum 2-3 years' experience of community health care experience
- Demonstrated ability to work with clients who live with multiple challenges related to marginalization, mental illness, substance use, complex health challenges, street level sex work, poverty, homelessness, and trauma
- Extensive knowledge of community resources, service & support programs, and barriers clients face in accessing same
- Working knowledge of a variety of treatment, intervention, and support strategies
- Computer literacy
- Master's Degree
- Suboxone on-line education program
- Experience in addiction medicine and harm reduction
