The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access safety, basic needs, health care and other mental health and addiction services under one location.

WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work - 70 hours over a two-week period; Nights including weekends
- What your benefits will be - 9% in lieu of benefits
- Your pension plan – Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)
HOW YOU WILL BE MAKING A DIFFERENCE:
- Provide a welcoming, safe, and trusting environment for vulnerable individuals through intake, screening, assessment and treatment planning, income and employment support and supportive housing.
- Support clients on their journey towards physical/mental health improvement, providing integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation and safety.
- Ensure client's basic needs are being met (eg. provision of food, showers, hygiene and a place to rest).
- Provide referrals to other supports and resources to maintain overall wellness.
- Assist with the development of care plans, navigating local services, and engaging in proactive crisis intervention.
- Provide one-one-one and group counselling sessions with a strengths-based wellness approach.
- Provide navigation assistance, connection to resources, and collaboration with established justice supports for those involved with the criminal justice system (pre-release support, probation or bail).
- Assist with transportation needs upon intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support.
- Provide accessible options for communication for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation and referral to language classes).
- Ensure client record is updated as per organization standards, discipline-specific guidelines and standards and PHIPA legislation, and handled in accordance with applicable privacy laws and organization policy.
- Provide support for additional HUB sites as required.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
- Post-secondary education in a related field of study. (Primary proof of credentials will be required during the hiring process, i.e. an original degree/diploma/certificate or official transcript; for ongoing education, an unofficial transcript will be accepted)
- Valid driver's licence and access to a vehicle with adequate insurance to carry out job duties/transport clients
- Ability to attend work regularly
We prefer the following qualifications:
- Current certification in Non-Violent Crisis Intervention (NVCI), ASIST Suicide Intervention, and/or Standard First Aid
- Experience working with individuals with addictions or problematic substance use; mild, moderate and serious mental illness; no fixed address
- Residential/community housing experience and familiarity with the Residential Tenancies Act.
- Experience in crisis intervention/management and crisis resolution.
- Experience working with individuals with an ABI or Developmental Disability.
- Experience working in a community based setting; knowledge of community resources.
- First Aid & CPR, ASIST, and CPI certifications
