(Includes supporting some residents who have a dual diagnosis, exhibit expressive behaviors and/or who are physical or sensory challenged, or technically dependent.)

Under the general supervision of the Site Supervisor/Area Director, the incumbent will primarily provide individually focused community and hub-based participation opportunities for older adults who have developmental/intellectual disabilities and who may also have expressive behaviours or sensory challenges and/or medical (dementia or other) or attendant care needs.
SKILLS, ABILITIES & QUALIFICATIONS, ETC. (as per Article 12.02):
Duties include, but are not limited to, taking a lead role in the
- Development and implementation of Individual Support Plans which promote independence, community immersion and natural support through personalized goal setting and achievement.
- Developing supports and approaches that take into consideration individual needs as one ages.
- Planning, coordination and facilitation of day-to-day activities (in the community and in-house) that will ensure persons have memorable & meaningful days that are filled with interest, variety, and choice.
- Cold-calls and recruitment of various stakeholders (participants, speakers, volunteers or placements, etc.)
- Creating connections within the community that promote independence, self-esteem and self-confidence of participants, as well as involvement of family, friends, volunteer and natural support.
- Providing coaching in varying community and volunteer settings and follow-up/maintenance supports
- Development/adaptation, coordination, and implementation/facilitation of new and interesting curriculum, sessions and courses geared to specific, varied audiences.
- Communication and follow up with family, consultants, medical personnel, support team, etc.
- Meeting the support requirements and personal care needs of older individuals, including those who may demonstrate high medical and/or behavioural needs.
- Thorough and consistent completion, submission and follow up of documentation and records.
- Modelling for/provision of guidance for new and existing team members, student placement and volunteers.
- Advocating for and ensuring the personal rights, dignity, well-being and protection from harm of participants, and reporting infractions
- Ensuring appropriate action in the event of illness, injury or critical situations
- Being respectful and considerate of others to create a positive work and support environment

- Proven leadership, initiative, creative thinking, problem-solving, and decision-making skills.
- Demonstrated excellence in curriculum writing, session planning, and group facilitation.
- Demonstrated excellence in collaboration and teamwork skills.
- Demonstrated positive attitude and resilience to intense job demands.
- Knowledge and experience using Microsoft Office and the ability to learn new software quickly.
- Demonstrated excellence in verbal and written communication
- Demonstrated understanding of adherence to and valuing of CLSL policies, procedures, performance standards and confidentiality requirements
- Willingness to work a variety of hours, as per individuals needs. Could include some evening and weekend hours.
EDUCATION AND EXPERIENCE:
- Minimum 2-year Post-Secondary Education in Developmental or Human Services REQUIRED.
- Additional experience or certification in gerontology and / or adult education an asset.
- Minimum two to three years experience in various support settings with proven ability to work well with persons who may have expressive behaviours and/or have medical and attendant care needs.
- Additional experience working with senior populations will be an asset.
- Current Standard First Aid and C.P.R. certificate; Current C.P.I. Certification.
- Valid Drivers License and use of a vehicle for transport of individuals as necessary.
