The Opportunity

Our vision is to serve the community by responding to unique and pressing health needs in our region by improving health outcomes for all individuals at all stages of life. YMCAs are uniquely positioned to partner with health practitioners and community health and wellness providers to deliver community-based health programs. The Community Health Manager will support and create programs to support individuals in preventing disease, managing chronic health issues, and transitioning to and from clinical care with a focus on physical activity. This inclusive approach considers social determinants of health, which significantly impact individual and population health.

The Community Health Manager is a member of the Health and Fitness Team at the Saint John Regional Y, reporting to the GM of Health, Membership and Recreation. This role will support the GM in contributing to the overall health initiatives at the YMCA of Southwestern NB and across NB.

Responsibilities

  • Lead the development of a referral system, intake process, and programs that will create a holistic system that leads to lifelong focus on a person's health.
  • Work with the individual YMCA fitness professionals and program managers to develop in-person, virtual and/ or blended exercise and education programs to support participants managing chronic health conditions and transition to appropriate YMCA general programming.
  • Lead provincial conversations with YMCA leaders about shared objectives and approaches to community health programming;
  • Coordinate shared approach to measurement and evaluation across NB community health programs, allowing for application of learning across programs and roll up of impact data.
  • Collaborate in the Y network to continue to develop best-in-class programs, share key learnings and provide development opportunities.
  • Develop and deliver exercise programs for participants and individualize programs based on needs.
  • Support leadership training to volunteers and staff to ensure program quality.
  • Develop and strengthen external relationships with various stakeholders, including referral partners, government representatives, and funders.
  • Develop systems and reports for program evaluations, pre and post intake.

Skills and Qualifications

  • CSEP- Certified Clinical Exercise Physiologist or Bachelor of Kinesiology
  • Experience leading a team of fitness professionals
  • Experience conducting individual fitness evaluations, health history evaluations and program evaluations
  • Experience recognizing, assessing and supporting marginalized populations through a health promotion and social detriment of health lens
  • Excellent oral and written communication and counseling skills
  • Excellent organizational skills and comfortable working in a team with healthcare professionals and community administrators
  • Bilingualism considered an asset
  • Flexible working hours include early mornings, days, evenings and weekends. The Community Health Manager will also be part of the HMR Support Team ensuring the YMCA of Southwestern NB Member Centers have support at all times it is open. This, along with supporting the GM will require the Lead to be available to work at any point of time during the operating hours.
  • Must have current CPR and First aid certification
  • Employment is contingent on having a clean Criminal Record Check.
