Department: South West Self Management Program

The South West Self Management Program (SWSM) is one of 14 regional programs across Ontario that provides a coordinated approach to support clients, caregivers and health care providers with self management. The South West region covers a large geographic area including the counties of Grey, Bruce, Huron, Perth, Oxford, Elgin, London/ Middlesex.

The community engagement and media specialist will support the South West Self Management program in connecting community individuals and health providers with self management supports and programs.

The SWSM program is housed within The Thames Valley Family Health Team which is one of the largest Family Health Teams (FHT) in Ontario. With more than 119 physicians across 19 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care to over 162,000 patients.

Duties & Responsibilities

  • Connect with Healthcare and Social Service Organizations: Forge connections with healthcare, social service and community organizations throughout the South West region.
  • Participate in Community Health Fairs and Networking Groups.
  • Foster relationships and collaborations within the community.
  • Social Media Marketing: Develop and implement social media marketing campaigns. Manage South West Self Management's online presence, including community event pages.
  • Content Creation: Manage and create content for South West Self Management newsletters. Target audiences include both consumers and health providers.
  • Facilitate community presentations in-person and online.

Qualifications and Experience

  • Post-secondary degree in Communications, Media, Public Relations, Journalism, or other relevant healthcare experience.
  • Solid understanding of marketing strategies and social media platforms.
  • Comprehensive knowledge of all players within the healthcare system would be an asset.
  • Ability to develop and facilitate impactful community presentations.
  • Ability to work independently and self-direct.
  • Strong verbal and written communication skills.
  • Comfortable traveling throughout the region, including overnight stays.
  • Excellent computer skills including: Microsoft Office (Teams/Sharepoint, Word, PowerPoint, Excel, Outlook, Zoom); Social Media (Facebook, Instagram, LinkedIn); Adobe; eNewsletter platforms; and similar applications
  • Demonstrated ability to engage and communicate clearly with a wide variety of audiences – including physicians, patients, caregivers, community agencies, as well as groups who are marginalized.
  • Experience developing tailored strategies to engage and communicate with diverse communities.
  • Access to a reliable vehicle.

Work Details:

  • Some evening and weekend work will be required.
  • Candidates should be able to carry supplies, set up displays, and comfortably work in front of a computer.
  • The role allows for flexibility to work from home. In office work may also be required.
