About the Job

Satori Health has a unique career opportunity for an experienced Licensed Practical Nurse to support patients as a Health Plan Coordinator. At Satori, Health Plan Coordinators act as patient navigators, effectively communicating health updates and working collaboratively with patients' healthcare teams to ensure they are fulfilling their long-term health goals.

The ideal candidate for the Health Plan Coordinator role is an LPN with over 5 years of established and diverse practice experience, a dedicated professional who is passionate about patient education and proactive healthcare. In this virtual setting you will navigate several technology platforms and systems to support your work with patients so experience and comfort with technology is necessary. Working with Satori Health

In this position you will have the opportunity to work from home and have in-office days with a collaborative and professional team of nurses. This hybrid casual role offers a fully flexible schedule (daytime, evening and weekend) .

Responsibilities 

  • Provide informative and engaging patient education to ensure patients feel comfortable, supported and well informed.
  • Guide patients through their health care support systems and set goals for optimal health care provision.
  • Conduct comprehensive review of patient care and history in EMR and Netcare with a strong focus on practitioner notes and relevant documentation such as lab work, diagnostic reports, consults and past medical histories.
  • Communicate and collaborate regularly with Practitioners to optimize patient support.
  • Contribute to team goals, consistently striving for excellence in outcomes.

Requirements 

  • Licensed Practical Nurse in good standing with CLPNA
  • 5+ years of diverse experience in various disciplines of nursing including critical care, urgent care, pediatrics, women's health, mental health, and/or chronic pain.
  • Experience working with varied chronic illnesses, critical care, internal medicine, and/or specialty practice.
  • Three or more years of experience working in a clinic and/or family practice setting considered an asset.
  • Sound knowledge of medical terminology, pathophysiology and reviewing lab and diagnostic results.
  • Intermediate to advanced knowledge of Netcare and an EMR system including Accuro, Med Access, Health Quest and/or Telus CHR.
  • Strong English language skills necessary for clear communication in written and verbal interactions related to job responsibilities.

  • Ability to work effectively both independently and collaboratively within a team.
  • Adept at communicating complex information with clarity and precision, both orally and in writing.
  • Proficient in MSOffice Suite with willingness and ability to learn new IT software applications.
  • Demonstrated ability to effectively manage and resolve conflicts.
  • Flexible and adaptable to shifting priorities and requirements.
  • Strong analytical skills with the ability to think critically and independently.
  • Able to work in a fast-paced environment with a high degree of accuracy and attention to detail.
  • Previous experience with virtual healthcare consultations an asset.
