We serve a remarkable community of individuals with diverse backgrounds, experiences and needs. Since founding in 1987 as part of the Welcome Home program, we've been dedicated to supporting adults with intellectual disabilities and co-occurring challenges. We provide a range of support models to meet the needs of individuals with Intellectual disabilities, Autism Spectrum Disorder, FASD, Mental Health challenges, problematic substance use, behavioural challenges as well as physical disabilities, ensuring each person receives the care, respect, and opportunities they deserve. Job Purpose:

Taking a leadership role within the support team, the case worker is responsible for providing direct support, assisting to identify and implement services and care for individuals supported, completing required follow-up, and contributing to individual and program planning. Collaborating and communicating with team members, families, stakeholders, and collateral agencies, the case worker helps to ensure that individuals supported by Simaril receive quality care and service.

Duties and Responsibilities:

General:

  • Maintain positive and effective communication.
  • Maintain positive public relations with community, family and other collaterals.
  • Follow and maintain policies and procedures regarding confidentiality and privacy when sharing information about people supported, employees, and Simaril Inc.
  • Understand and conduct yourself according to Simaril Inc. policies, procedures, and legislative requirements.
  • Provide resources for relief and replacement, including Emergency On-Call duties and support through crises when required.
  • Report any agency-related changes and significant issues to the Case Manager or Program Manager.
  • Actively engage in team development within the organization and professional development.

Direct Support (Program Support):

  • Maintain regular communication with the individuals supported, team members, families, and external agencies regarding the needs and desired outcomes of people receiving support.
  • Maintain regular contact with individuals supported and advocate for those supported when necessary.
  • Actively participate in the planning process by identifying areas for service growth and future development.
  • Ensure that services are working towards achieving desired outcomes and goals of people supported.

Administration:

  • Ensure record keeping is current and secure in the appropriate locations as required, meeting all legislative and Agency requirements, including but not limited to personnel documents, supported individual documents, and meeting minutes.
  • Assist with monitoring the management of funds for people supported when needed.
  • Attend and participate in Leadership meetings as required.

Skills and Qualifications

  • 2-3 years of full-time Direct Support Work experience.
  • Leadership experience.
  • DCSP (2-year program) or related Post-Secondary education.
  • Proficiency in Excel, Word processing, Email, and internet.
  • Valid Driver's license and access to reliable vehicle preferred.
  • Reliable, responsible, and excellent interpersonal skills.
  • Ability to work in a busy environment and prioritize duties.
  • Well-organized, able to work independently and as part of a team.
  • Provide essential employment documentation, including Criminal Record Check and Adult Abuse Registry check.
  • Current First Aid Certification (level c).
  • Self-motivated with little external input.
  • Ability to work well in a team environment and understand the agency's vision, mission, and values.
  • Good judgment and flexibility in assessing and providing solutions to situations.

Hours of Work and Working Conditions:

This is a full-time position with hours reflective of the program needs, Tuesday-Saturday 10:00 am- 6:00 pm. To provide additional support outside of the standard work hours, you may be required to work some overtime hours with paid compensation.

Physical Requirements:

This job requires a lot of sitting, walking, bending, and lifting. The case worker is frequently exposed to noise and a regular flow of people around the office or at the work location.

Compensation and benefits:

The Case Worker's wage starts at $22.60 an hour.
