Hours: 20 hours per week, 9-5pm and one Saturday 9-3pm shift per month

About Us:

The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centered primary care with the goal of optimizing individual and family health within the community.

We provide primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in the northwest sub region of the greater Toronto area. The HRFHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care. About the Position:

As a key member of the Humber River Family Health Team (HRFHT), and an integral part of an Interprofessional health care team, the chiropodist will provide a full range of quality foot care to all HRFHT patients. The range of services includes active treatment of acute pathological foot conditions, surgical procedures, and preventive & palliative foot care.

Roles and Responsibilities:

Clinically:

  1. Assessment of foot disorders or dysfunctions and treatment of same therapeutic, orthotic or palliative means;
  2. Differentiating between disorders of the foot arising from systemic or general disease and those arising from local, structural and/or functional disorders;
  3. Recognizing patients having conditions beyond their scope of practice and refers to other appropriate health care professionals (e.g. primary care provider);
  4. Designing and implementing a management plan based no presenting conditions and the patient's individual needs. The Chiropodist will reinforce and encourage patient responsibility and participation in the management plan;
  1. Communicating foot health education in a variety of settings to patients, Interprofessional health care professionals (IHPs) or other concerned individuals;
  2. Ensuring appropriate antiseptic techniques, safe procedures and isolation practices are maintained in the clinical and/or surgical setting; and providing a safe environment for patients, practitioners and others in the clinic.

Administrative Duties

  1. Plans for future clinic needs to ensure that HRFHT clinical goals and objectives are met; and ensure that HRFHT mission and vision statement are at the core of service delivery.
  2. Ensures safe working practices and that all clinical equipment is in satisfactory operating condition;
  3. Maintains and upgrades knowledge and skills regarding new procedures, methods and equipment through attendance at relevant seminars and educational programs, prepares quarterly reports, and monitors daily statistical data;
  4. Prepares work schedules that ensure efficient use of time;
  5. Adherence to HRFHT policies and procedures
  6. Assist in the planning, implementation and evaluation of chronic disease and health promotion and illness prevention programs and services.
  7. Collection of data as required for statistical/reporting

Reporting to: Executive Director.

Qualifications and Competencies:

  • Sc. and licensing/registration by the College of Chiropodist of Ontario.
  • Additional training or knowledge in the areas of chronic disease management and geriatrics is considered an asset.
  • Demonstrated clinical skills in assessment, program planning, implementation, and evaluation.
  • Work effectively, both independently and as part of an inter-disciplinary
