YW Calgary is looking for an Intake and Assessment Coordinator to join our Team! Reporting to the Transitional Housing supervisor, the Intake and Assessment Coordinator team member provides a critical role in assessing and successfully accepting women for our Transitional Housing Program. This role works directly with potential clients for acceptance into the program or referrals to more appropriate community resources. The Intake and Assessment Coordinator is the entry point into the YW Calgary Transitional Housing Program. This role also provides case management to clients residing in the emergency bed.

This position is a Temporary full-time position until January 2025. This role works 37.5 hours per week 10:00 AM to 6:00 PM, Sunday to Thursday. This position is not eligible for a Hybrid work arrangement.

WHAT YOU'LL DO:

  • Complete intakes for women moving into Transitional Housing program and complete all appropriate documentation in a timely manner.
  • Facilitates screening meetings with prospective clients and assess for appropriateness into the program using program assessment measures.
  • Monitor and manage the program logbook and status sheets.
  • Conduct initial “meet and greet” tours for potential clients of Transitional Housing program.
  • Inform clients of their rights and responsibilities, including the complaints and grievance process, program expectations and other pertinent program guidelines while participating in housing services
  • Work with women to understand and develop goal plans aiming to resolve issues related to barriers to securing housing and update client's file and information accordingly.
  • Collaborate with other team members and actively network with community partners involved in shelter programing to assess housing needs and remain current in terms of processes and community resources to proactively help women maintain suitable housing
  • Demonstrate a commitment to personal and organizational continuous quality improvement by providing innovative ideas and develop possible alternatives that have the potential to improve the program.
  • Adhere to the record keeping and documentation standards/expectations of the program.
  • Maintain database of referral and client contact to ensure that intake process is being managed and that client weekly contact is being tracked.
  • Maintain and monitor the program logbook and (room) status sheet and conducts client assessments and surveys.
  • Keep accurate monthly statistics and ensure that client files are closed appropriately upon program exits
  • Actively participate in strategic program planning and quality improvement planning and implementation
  • Foster and contribute to a culture of inclusion and equity in a diverse work environment; understand and recognize that individual differences significantly contribute to YW Calgary's success in its programs and workplace.
  • Integrate diversity, equity, and inclusion principles into one's role, work, and engagement with others, as aligned with YW Calgary's values and Practice Framework.

  • Inform team members and leaders to stop any unsafe work practices immediately to create awareness and injury/illness prevention.
  • Report any hazards or incidents immediately to your leader in accordance with YW Calgary's incident reporting requirements to help ensure pro-active risk mitigation and management in compliance with legislative requirements (Occupational Health & Safety, Workers' Compensation Board, etc.)
  • Use all safety materials, equipment, devices, and tools required to safely perform work in alignment with YW's role and task requirements.
  • Share individual accountability to create and maintain an inclusive environment the fosters a physical and psychologically safe workplace. Actively participate in the implementation of the National Standard for Psychological Health & Safety and integrate into all work practices and interactions with colleagues.

WHAT YOU'LL BRING:

  • A clear Police Information Check, including a Vulnerable Sector Search, and an Intervention Record Check is required upon commencement of employment
  • Bachelor's degree in social work, Human Services, or related field. Other educational backgrounds considered with related experience.
  • Active registration with a relevant professional association is an asset.
  • At least one year of experience working with vulnerable women populations experiencing complex mental health, addictions, family violence, trauma, and homelessness.
  • An understanding of addictions including withdrawal management, harm reduction, stages of change, trauma informed care and recovery-oriented systems of care.
  • Strong assessment skills including assessing and determining client needs and strengths, creating case plans from a client centered lens, accessing resources, and building informal supports.
  • Proficiency in the service delivery process: engagement; assessment and planning; intervention and termination.
  • Case Management expertise, knowledge and skills in crisis intervention, and conflict resolution is required.
  • Ability to assess complex clients with various needs, supporting and referring to appropriate support services within the community.
  • Excellent communication skills and the ability to provide exceptional service over the phone and in person.
  • Current ASIST, NVCI, Brain Story, First Aid / CPR and Naloxone Training are required.
