JOB SUMMARY

NB Health Link (NBHL) leverages technological solutions and innovative clinical delivery models to provide access to primary care services either virtually or in person based on the unique needs of the patients.

Reporting to the manager, the LPN is part of the NB Health Link team. The LPN is an essential team member who ensures the proper record keeping of the patient's medical chart, supports the providers in the clinical setting to augment capacity and works with the multidisciplinary team to assure optimal outcomes for patients.

ROLES AND RESPONSIBILITIES

The NBHL LPN will provide a wide range of clinical support services to the NBHL program, for which they have been trained and is within the LPN scope of practice. Duties include but are not limited to:

Recording and updating the medication list on the patient's profile and contacting the patient, as needed, to ensure the best possible medication history.
Assessing patients and gathering their complete medical history.
Assisting the provider in the clinical setting with tasks such as taking measurements (VS, height, weight, etc.) and collecting specimen samples.
Collaborating with the interdisciplinary team to develop the plan of care.
Providing patient education.
Maintaining stocks of supplies in clinics and checks for expiry dates.
Maintaining a clean and safe work area by cleaning patient equipment and exam rooms.
Assisting in orientation/education of new staff regarding the NBHL program
Other responsibilities assigned by the medical director or manager.

REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:

Registered in good standing with the LPN Association of New Brunswick (ANBLPN).
BCLS certification and maintain competence.
Ability to work effectively with a multidisciplinary team.
Able to work independently.
Must have a high regard for confidentiality.
Strong computer skills using a variety of Microsoft Office software products as well as the Internet and scheduling software.
Well organized with the ability to prioritize workload in a fast-paced environment.
Excellent communication skills both oral and written.
Comfortable working to a schedule with demanding turn-around times.
Ability to work in a remote working environment.
