Definition of Work: The primary function of the Social Worker Assistant is to assist the Social Worker in the day-to-day provision and coordination of activities necessary for effective case management. The Social Worker Assistant is responsible for the provision of distinct administrative and supportive services to clients of social workers receiving interventions and services from Western Health. These activities include, but are not limited to, preparing financial requests, soliciting information on behalf of social workers; arranging/organizing supportive services; and providing supervision of clients, as per program policies and standards, as required by the social worker.
Required Qualifications: Graduation from high school, supplemented by three (3) years of post-secondary education in the social science field; a minimum of once (1) year experience working with clients and families; a thorough knowledge of inpatient and community programs and services; a working knowledge of Microsoft Office (particular Microsoft Word) is essential. Effective time management, communication, organization, and interpersonal skills are essential. Ability to manage conflict and problem solve as well as collaborate with teams is required. Applicants must also be prepared to demonstrate an ability to be self-directed and exercise independent clinical judgement in an acute care environment with limited supervision. The incumbent is expected to work collaboratively with team members and community partners to assess the needs of the patient; develop and implement care plans and support transitions in care. A valid driver’s license is required; Certificate of Conduct, Vulnerable Sector Query satisfactory to the Employer.
