Job Summary
Reporting to the Senior Friendly Care Manager – Community, the Social Worker II (SW
II) is an advanced professional position requiring a high level of clinical expertise,
judgment, and independence. The SW II provides leadership and guidance to program
staff, clinical coordinators, and management, while delivering comprehensive social
work services to older adults within the Geriatric Community Outreach Program. In this
role, the SW II collaborates closely with interdisciplinary teams and community partners
to ensure coordinated, person-and family-centered care. The SW II also serves as a
consultative resource, offering expert social work support and contributing to the
development and implementation of best practices in the care of older adults across
community settings.
Within policy parameters, and in collaboration with the Geriatric Community RN , the
SW II acts as a resource and provides orientation support, clinical consultation and
advice, direction and assistance to community support staff and clinical coordinators.
Duties include but not limited to the following;
*Coordinate services with healthcare providers, community agencies, and other service
partners.
* Develop, coordinate, and oversee the clinical orientation of community support staff in
the care of older adults;
* Provide and oversee opportunities for peer support in clinical practice;
* Develop and deliver educational and communities of practice opportunities to clinical
staff and coordinators on mentoring / coaching activities to support staff development;
* Support staff in their professional / practice development
*Assist in the development and evaluation of special programs and initiatives to meet
the needs of older adults
* Provide education and consultation to clients, families, and community stakeholders
on geriatric care.
* Assess clinical competence and implement strategies to promote clinical excellence;
* Conduct best practice research in geriatric care; develop and facilitate education to
improve clinical assessment skills and evidence-informed practice.
* Travel within the zone may be required
As part of the Geriatric Community Outreach Programs team the SW II is responsible
for leading program initiatives, collecting and analyzing workload data, coordinating
geriatric education and orientation to staff, participating in regional and provincial
committee work, supporting policy and program development and liaising with both
internal and external partners and other health professionals. The SW II assesses staff
learning needs, identifies practice trends, and contributes to program evaluation and
continuous quality improvement.
The SW II demonstrates understanding of and promotes geriatric clinical best practice
throughout all levels of the organization. The SWII ensures that services are delivered in
accordance with the Canadian Association of Social Workers Code of Ethics, the
Standards of Practice in Social Work, and the Social Workers Act of Newfoundland and
Labrador. The SW II promotes self-determination, empowerment, and community
inclusion in practice, and is committed to person- and family-centered care while
identifying and addressing risks to enhance client and staff safety.
Job Qualifications
-Graduation with an accredited Bachelor of Social Work Degree is required.
-Current and continuing registration with the Newfoundland and Labrador College of
Social Workers (NLCSW) is required.
- A minimum of three years' experience within the last five working as a Social
Worker in community support services
- Demonstrated knowledge of the scope and complexity of the care of older adults
- Knowledge of relevant legislation, including but not limited to, Mental Health Care
and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act,
Medical Assistance in Dying, and the Children, Youth and Families Act.
-Demonstrated knowledge of the Momentum InterRAI Assessment tools, Provincial
Home Support Program, and Adult Protection.
-Considerable knowledge of the needs, resources and services available to older
adults
-Demonstrated ability to work well independently and with others in an
interdisciplinary team environment.
-Demonstrated ability in completing comprehensive assessments, interviewing and
crisis intervention techniques, and social work programming and delivery methods.
-Previous leadership experience is an asset.
-Demonstrated strong program development and evaluation skills.
-Demonstrated analytical, critical thinking, problem solving, decision-making and
organizational skills.
-Demonstrated strong written and oral communication skills as well as the ability to
deliver/facilitate effective presentations.
-A satisfactory record of work performance is required.
-Demonstrated skills in computer, electronic charting and database management is
required.

-Use of a private vehicle is required to visit clients in their home setting.
