CASE MANAGEMENT COORDINATOR
JOB DESCRIPTION
OVERVIEW
The Case Management Coordinator (CMC) plays a key role in leading the coordination and delivery of trauma-informed, feminist, client-centered support to women, children and families accessing Liberty Lane's second-stage housing and outreach services.
Reporting to the Program Director and working in close collaboration with the Domestic Violence Project Coordinator, the CMC leads the delivery of services and programming within Liberty Lane's Family Stream, including children's programming and services for individuals within residential and outreach programs.
The CMC is responsible for a small individual caseload (approx. 6 clients per month) while also providing case management consultation to other members of Liberty Lane's second stage and outreach service delivery team.
In addition, the CMC works collaboratively with the Domestic Violence Project Coordinator to supervise the Family Case Workers, Child and Youth Workers, volunteers and students who compose Liberty lane's second stage and outreach service delivery team.
KEY RESPONSIBILITIES
Coordination and Supervision
- Collaborate with the Program Director and Domestic Violence Project Coordinator to provide general supervision and support to members of Liberty Lane's second stage and outreach service delivery team
- Serve as the direct supervisor for up to 4 Case Workers, including
- holding monthly 1:1 meetings with assigned staff to monitor workplans, well-being, caseloads and provide assistance in navigating any other general supports required by Case Workers
- maintaining meeting minutes and related documentation to support performance management
- providing general input and guidance on case management processes and client work in collaboration with the Domestic Violence Project Coordinator, Program Director, and other external supports as needed
- Support the Program Director in planning and facilitating monthly Program meetings, including maintaining up-to-date, accurate meeting records and following up on action items
- Identify and highlight opportunities, challenges, or issues of concern related to case management and/or programming to the Program Director
Case Management and Client Support
- Collaborate with the Program Director and Domestic Violence Project Coordinator to maintain and monitor client inquiries for services, waitlists, and case assignments
- Conduct intake, assessment and individualized case planning for Liberty Lane's Family Stream
- Attend monthly Coordinated Community Response meetings as needed
- Provide one-to-one crisis and long-term support for residential and outreach clients and their children within the Family Stream (approx. 6 clients), including:
- supporting adult clients in goal setting related to safety, housing, parenting, employment, and wellness
- supporting parents with goal setting related to safety planning, housing, parenting challenges, employment and general wellness
- supporting the children of survivors to enhance emotional regulation, social and coping skills within a child centered, play-based framework
- facilitating referrals to community resources, including counselling, education, and legal support
- assisting clients with applications for housing, financial assistance and other supports
- collaborating with schools, child welfare agencies and other community resources
Administration and Other Duties
- Maintain accurate and confidential records, case notes, statistics, and other necessary documentation in accordance with Liberty Lane's requirements
- Prepare regular reports for the Program Director, Executive Director, Board of Directors and others as needed
- Participate in staff meetings, supervision, training, and professional development
- Assist the Program Director with human resources management, strategic development and other initiatives as needed
REQUIREMENTS:
- Ability to work in-person at Liberty Lane's Fredericton office
- Post-secondary degree in Social Work, Psychology, Counselling, or a related field, or equivalent combination of training and experience
- Minimum 2 years of experience in a relevant professional role. Experience working with survivors of domestic violence or family support services or another community service setting considered an asset. Direct experience working with children aged 0-12 in a professional or volunteer capacity
- Strong foundational knowledge base of the following:
- dynamics of intimate partner/domestic violence, and the effects on victims / survivors and their children
- trauma, mental health and addictions
- principles of trauma-informed care
- basic counseling skills and intervention strategies
- Experience and/or training in clinical supervision considered an asset
- Demonstrated experience in supervising or managing teams
- Strong organizational, communication, and crisis intervention skills
- Ability to work independently and as part of team
- Valid driver's license and car access to allow for travelling to various community locations
- Successful background checks (i.e. Criminal Record & Vulnerable Sector Background Check from Police, Prior Contact Check from Social Development)
- Flexibility to work limited hours on occasional evenings or weekends.
ADDITIONAL EXPERIENCE CONSIDERED ASSETS:
- Professional, clear and effective written and verbal communication skills
- Excellent interpersonal skills to work with clients and community members in both individual and group settings.
- Excellent organizational skills, including ability to manage multiple projects while maintaining keen attention to detail.
- Ability to empathize while setting strong boundaries.
- Healthy capacity for problem-solving, responding to situations at a moment's notice, and remaining calm in crisis.
- Valuable team player, who can also be counted on to work independently and be self-directed.
- Competence in using relevant software applications, including Microsoft 365, Google Workspace, Zoom, and Teams
- Familiarity with a broad range of local resources in the greater Fredericton area.
- French or other second language.
EMPLOYMENT DETAILS
This is a full-time 37.5 hours/week, permanent position based in Fredericton, New Brunswick, with regular on-site work at our offices and throughout the surrounding community. A valid driver's license and access to a vehicle are required.
The salary range for this position is $54k - $61k per year, based on experience and qualifications.
