HOMEBASE is an Integrated Youth Services (IYS) model of care and part of a global movement focused on transforming how youth, young people, and their families access the resources, services, and supports they need. The primary goal of HOMEBASE is to support communities in connecting youth to the right services, in the right place, and at the right time, with a strong emphasis on collaboration, multidisciplinary care, and early intervention.

The Community Paramedic (CP) plays a key role in promoting health and preventing illness and supplements existing Primary Health Care services. This role is responsible for identifying health needs and risks, developing and implementing health programs, providing direct patient care, and collaborating with other healthcare professionals to improve access to care and health outcomes for youth in the community.

As part of the Mobile Integrated Health Team, the Community Paramedic works closely with YMCA Homebase staff and supports a diverse patient population, including youth requiring mental health and addiction services, physical health supports, and coordination of care and community-based services. The CP works collaboratively within an interdisciplinary framework alongside service providers such as Community Mental Health and Addictions, Primary Health Care, Psychiatry, Paramedic Services, justice and victims' services, child and family services, police services, and other community partners.

As an employee of Medavie Health Services (MHS), Community Paramedics are accountable to patients and coworkers by actively participating in and supporting all safety-related initiatives and fostering a culture that prioritizes patient safety and a safe, respectful workplace.

The position entails, but is not limited to the following:

  • Independent primary clinical care.
  • Maintain and operate ambulances and related equipment in accordance with patient needs and public safety.
  • Clinical expectations and responsibilities for Primary Care Paramedics are based on the current National Occupational Competency Profile (NOCP), the Saskatchewan College of Paramedics registration requirements and Medavie Health Services West policies, procedures, and protocols.

As the ideal candidate for this role, you will have:

  • Must meet all legislated requirements for licensure in jurisdiction of employment
  • Must be currently registered with the Saskatchewan College of Paramedics as a Primary Care Paramedic or eligible for registration through the Saskatchewan College of Paramedics. Candidates with restricted licenses will be considered, with preference to those with unrestricted licenses.
  • Must be in good standing with the Saskatchewan College of Paramedics .
  • Must submit with resume a copy (front and back) of a valid Saskatchewan Class 4 Driver's License (or equivalent).
  • Must submit with resume a current ITLS and CPR C for Health Care Providers.
  • Must submit with resume a current (within last 60 days) valid and clean criminal record check.
  • Must submit with resume a current (within last 30 days) driver's abstract.
  • Must demonstrate the ability to work efficiently as a member of the team and possess strong interpersonal skills.
  • Must be able to work all shifts as assigned, maintaining a good attendance record.
  • Must have a high regard for confidentiality.
  • Must have excellent work habits and exhibit professionalism
  • Be physically capable of performing the required duties;
  • Proficiency with using electronic medical record system and patient care reporting;
  • Provide proof of adequate antibody titers for the following diseases: Diphtheria, Tetanus, Measles, Mumps, German Measles and Hepatitis B. Results of a Mantoux test are also required;
  • Experience working with vulnerable populations in the community;
  • And experience working with persons with disabilities
  • Demonstrated ability to provide initial psychological interviews and assessments; and
  • Experience on interdisciplinary teams, in program development and case management;
  • Community care experience an asset;
  • Willingness to participate in professional development and education related to the role is considered an asset;
  • Knowledge of local community health care needs, knowledge of facilitators and barriers to local population wellness;
  • Graduation from a recognized Community Paramedic Program or willingness to take training is a requirement;
  • Nonviolent Crisis Intervention (NVCI) training is considered an asset;
  • Core Partner Child Safe Guarding Training mandatory (online training)
  • Suicide risk assessment training is considered an asset
  • Violence risk assessment training is considered asset;
  • Knowledge of computer programs (Microsoft Office Suite & Teams, Google Drive etc.) is considered an asset;
  • English Required; French not required but would be an asset
  • Demonstrated ability to provide initial psychological interviews and assessments;
  • Experience on interdisciplinary teams, in program development and extensive case management;
  • Requires independent critical thinking, clinical judgment and independent decision-making skills required to develop, implement, and monitor care plans;

The hours of work will be up to 16 hours per week. The work environment involves indoor. Strenuous physical movements are spent standing, sitting, kneeling crouching, bending, and lifting and / or carrying objects. Required to maintain one position for possible long periods of time, such as sitting at meetings, or at a desk setting. May be exposed to patients with communicable diseases. May encounter violent patients. Irregular working schedules, shift work, standby and extended overtime as needed. Works without immediate supervision. The employee will exercise a high degree of discretion and independent judgement. Regular and irregular business hours are to be expected. The incumbent may be needed to fill in for sickness, vacation etc.

Employees will be reading data or inputting data, report writing, operating a word processor, computer, calculator, and talking on the telephone. Use and exposure to general office equipment such as fax, photocopier, printer, and personal computer. The employee will perform duties in relation to their ability and scope as a licensed Paramedic. It is expected that any MIH position, including the Community Paramedic position, may be called upon to perform these duties at any point throughout a shift.
