Definition of Work:
This is an advanced professional level position involving a high degree of responsibility, autonomy and independence in identifying and providing specialized assessment, individual service planning and critical intervention services.
The Mental Health Case Manager will coordinate care for clients with severe and persistent mental illness. The role will involve case management and direct intervention for clients discharged from the Psychiatric Unit at the Central Newfoundland Regional Health Centre and other mental health clients as referred and appropriate for services. The Case Manager will assist clients to maintain wellness and enhance their ability to function in their homes and communities. The successful candidate will be expected to contribute to and be supportive of interdisciplinary team goals and objectives, research, and evidence-based best practice. Through the application of high level interpersonal relationship and advocacy skills, he/she will work with clients, interdisciplinary teams, and community organizations to strengthen/expand the clients' resource base and their ability to access that resource base. In situations where resources are either lacking or unavailable to clients, the Mental Health Case Manager will function assertively as a client advocate.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
? Undergraduate degree in nursing or a Registered Nursing Diploma from an approved school of nursing, supplemented by a Certification in Mental Health/Psychiatric Nursing from CNA.
? Current registration with the College of Registered Nurses of Newfoundland and Labrador.
? Significant (2-5 years) and current (within the last 5 years) experience in a mental health and/or addictions setting.
? Current BLS certification for health care providers from the Heart and Stroke Foundation.
? Demonstrated exceptional skills in assessment and crisis intervention.
? Demonstrated understanding and commitment to person and family-centered care in the delivery of services.
? Proven ability to work with a high degree of independence and initiative while collaborating with an interdisciplinary team.
? Demonstrated knowledge of the DSM-5.
? Proven proficiency with computer systems such as CRMS.
? Demonstrated knowledge of programs, services, and community resources.
? Demonstrated strong skills and abilities in areas of interpersonal relationships, interdisciplinary team collaboration, leadership, communication, advocacy, networking, time management and client support.
? Knowledge, skills, and experience should demonstrate consistency with community mental health and case management principles and practices.

? A satisfactory record of work performance and attendance is required for this position.
? A satisfactory Certificate of Conduct from the RCMP or local police authority.
