Job SummaryThis is a challenging and highly self-directed professional position, requiring minimal supervision by the Manager of Mental Health and Addictions Services, Rural Eastern. The Mental Health Nurse Case Manager applies their extensive knowledge and understanding of community Mental Health and case management models, roles and function, psychiatric conditions (medical and psychosocial models), scope of professional practice, standards and ethical guidelines in coordinating and delivering the full range of case management services to individuals with severe and persistent mental illness. The incumbent must be an experienced mental health clinician, both familiar and comfortable with working in non-clinical settings such as community environments and individual clients’ homes. The incumbent contributes to and is supportive of interdisciplinary team goals and objectives, research and evidence-based practice. He/she conducts assessments and helps clients grow and feel comfortable/secure in their communities and works with interdisciplinary teams and community organizations to strengthen/expand the clients’ community resource base and their ability to access that resource base. Where needed (resources are either lacking or unavailable to clients), the incumbent will function assertively as a client advocate.

Constantly uses fine finger/precision, auditory, sensory and visual concentration to provide and conduct assessments, observe/monitor clients, provide feedback to clients and other health care providers and complete documentation.
A high level of attentiveness and vigilance is required when working alone and during counselling sessions to ensure safety or detect impending danger/harm to self or client.
Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing.
A minimum of three out of the last five years’ experience in mental health nursing.
A current practicing license with the Association of Registered Nurses of Newfoundland and Labrador (ARNNL) and BLS Certification is required.
The successful applicant will be self-motivated and able to work both collaboratively and with a high degree of independence and initiative.
A valid Newfoundland and Labrador Class 5 Driver’s License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance is required.
