Long-Term Care, Primary Care for the Fogo Island Health Services Area

Definition of work: The Nurse Practitioner (NP) role is responsible to provide comprehensive assessment and primary care to all clients within the Health Services area. This is an advanced practice role that will provide an autonomous, professional, analytical and comprehensive range of health services to clients and patients in various settings such as LTC, Primary Care and community health, utilizing the Home First Approach. These services include but are not limited to health assessments and diagnosis of diseases, disorders, injuries and conditions; treating and managing acute and chronic illnesses and identifying health risks and needs that exist within the Fogo Island/Change Islands Health Services Area.

This NP role will provide advanced nursing care including health promotion and prevention and chronic disease management and education in collaboration with physicians and other members of the multidisciplinary team. The NP will work in partnership with the multidisciplinary team and via various committees that are inclusive of island residents to develop, lead, implement and evaluate best practice. The NP will work with the Clinical Nurse Educator/Facilitator in the development and evaluation of policies, standards of care and clinical services for residents of Long-Term Care at the Fogo Island Health Centre. The NP will assume a leadership role in influencing clinical practice and patient outcomes and collaborate with the broader healthcare team and community partners regarding complex and challenging care situations within the Fogo Island Health Services Area. The NP will promote a person and family centered philosophy to care and establish a preventative, integrated, approach that is driven by individualized assessment of needs and individual choice.

The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
* Graduation from a nurse practitioner program approved by Central Health.
* Current registration as a nurse practitioner with the Association of Registered Nurses of Newfoundland and Labrador.
* Current BLS certification for health care providers from the Heart and Stroke Foundation.
* Gentle Persuasive Approach or willingness to complete next available course.
* CNA certification in Gerontology or willingness to complete next available course.
* Demonstrated knowledge of home and community health services.
* Demonstrated exceptional leadership qualities and problem-solving skills as well as oral and written communication skills.
* Demonstrated ability to work well independently and in an interdisciplinary team environment.
* A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
* Consistent demonstration of Central Health's corporate values; Innovation, Compassion, Accountability, Respect, and Excellence (I CARE)
* A satisfactory record of work performance and attendance is required for this position.
* A satisfactory Certificate of Conduct from the RCMP or local police authority.

Preferred Qualifications:
* Demonstrated skill in using health care-based computer systems including CRMS and Meditech.
* Completion or willingness to complete next available course in ACLS, PALS, TNCC and NRP.

Hours of work: 75 hours biweekly (On-Call/Shift work may be required)
Salary: NS-35 ($50.28-62.40)
