The New Brunswick Association for Community Living (NBACL) is leading in the creation of opportunities for people with an intellectual or developmental disability to live full and valued lives in all aspects of society and is seeking a dynamic, energetic and enthusiastic individual committed to the vision, values and philosophy of inclusion. Under the direction of the Manager of Independent Facilitation and in accordance with the New Brunswick Association for Community Living (NBACL)'s contract with the Department of Social Development, the Independent Facilitator is responsible to assist individuals with disabilities aged 19-64, who are applying for supports through the provincial Disability Support Program (DSP) to:
- Identify their goals, requirements for disability related supports and services, and possible options to have supports and services provided, and;
- Prepare individualized disability support plans for consideration for funding by the Department of Social Development.
- The successful candidate will work in the greater Moncton area and be responsible for covering Region 1.
- Assist individuals to develop a vision for their future, using a person-centered approach while working collaboratively with their natural supports when available;
- Assist individuals to identify their needs for disability related supports as well as possible options for meeting those needs using both paid and unpaid sources of support;
- Verify that all potential resources and supports have been considered in the development of the proposed support plan;
- Ensure to the fullest extent possible that individual applicants understand the choices and options that are available to them and that their wishes and choices are being respected by others;
- Assist individuals in preparing an application for disability supports. This will include assistance with the development of an individualized disability support plan and funding requirements, as well as assistance with the completion of the DSP application form and any supplementary documents;
- Assist individuals in submitting their DSP application to the Department of Social Development for possible funding approval;
- Assist individuals, as part of the initial application process, to access any generic services or other government or community programs that may be required.
- Bachelor's degree in the social sciences, Human Service Counselor diploma, or combination of equivalent education and experience in the human service field;
- Valid driver's license, a reliable vehicle necessary for work related duties and the willingness and flexibility to travel throughout the regions to which assigned;
- Flexibility in working hours;
- Previous experience in working with persons with a disability, knowledge of inclusive practices and person-centered planning will be considered assets.
- Strong computer skills and demonstrated proficiency with Microsoft Office applications;
- Current and Valid Criminal Record Check and Vulnerable Persons Check;
Bilingualism (Written & Spoken French and & English) is considered a strong asset.
