We are looking for a motivated Community Connector Coordinator to join our Social Prescription, Community Health and Specialized Programs team!

SUMMARY:
The Community Connector Coordinator plays an integral role in bridging the gap between healthcare and social care services particularly for seniors residing in the City of Burnaby. As a social prescribing professional, the Community Connector Coordinator collaborates and partners the Seniors Services team and with community organizations to access and provide non-medical support to improve the overall health and well-being of older adults in the community. The goal of this position is to assist frail and isolated older adults who are 65+ to support them in healthy aging, and to continue to be independent by connecting them with social engagement and physical activity opportunities as well as providing tips for healthy nutrition and connections to community-based seniors' services in the Burnaby area.

POSITION FUNCTION:


1. Accountability
• Be familiar with MOSAIC'S vision, mission, values, strategic plan, and integrated client service delivery approach, program and/or project proposals and contracts, and your role in achieving the goals and contractual outcomes of the department.

2. Operations
• Facilitate educational training for seniors and community providers.
• Provide one on one information and education to seniors in co-developing their wellness plan and addressing social barriers and challenges.
• Wellness Plan Development:
• Supports clients to access an appropriate range of activities and suitable community resources by developing individual wellness plans.
a). Provides information regarding appropriate community resources to socially support the participant and their families.
b). Effectively collaborates within the Agency's and community's Seniors Services to provide multidisciplinary care for the best interest of the senior.
• Maintains an active client caseload and prioritizes referrals based on needs and communicates with health care professionals and community agencies as needed:
a). Work with seniors who are referred by the health care professionals and community partners to connect them with community supports and services through referrals, advocacy, introductions, and applications.
b). Connects, liaises, and establishes partnerships with local healthcare professionals to create and maintain referral pathways.
c). Use related databases and records to enter participants' information, document wellness plans, and progress reports, and other

administrative duties as required.
d). Provide follow-up/progress notes to health care provider as requested.

• Implement safety precautions when visiting clients in the community.
• Complete intake process to assess strengths, needs, abilities, and risks using motivational interviewing techniques.
• Maintain a high level of confidentiality in all matters related to clients and community partners.
• Provide support to volunteers:
a). Support the Social Prescription Team in all phases of volunteer engagement (onboarding, training and resources, and offboarding).
b). Help maintain the volunteer database and newsletters.


3. Community/Stakeholder Relations
• Builds and maintains networks within the community and strengthens relationships within the community-based senior-serving sector.
• Engages and participates in learning opportunities such as in community of practices, healthy aging topics, planning tables or networking events.
• Participates in evaluation of programs including collection of participant data, reporting at regular intervals learnings of program, and by attending communities of practices.

4. Reporting
• Assist the supervisor with the completion of required deliverables and funder reports by providing relevant data and information related to the project.
• Perform administrative duties related to the project and agency (e.g., processing invoices, and handling petty cash) as applicable.
• Participate in all relevant meetings, including internal agency meetings and meetings with project partners and/or stakeholders, and relevant training workshops and professional development opportunities.

5. Other Duties as assigned by the supervisor.

QUALIFICATIONS:

Knowledge

Seniors' needs and challenges related to healthy aging; social prescription, gerontology, seniors' care, community-based supports, social determinants of health, equity-centred design framework, health promotion, primary care, healthcare practices, health beliefs, Cultural sensitivity, language needs, and understanding of cultural nuances of older adults who are frail and isolated; knowledge of Burnaby's and Metro Vancouver's community resources, programs and services; MS Office and cloud-based database; standard office equipment.

Education

Post secondary degree, certificate, or diploma in social services, including but not limited to Social Work, Gerontology, or Psychology, Medicine, or Nursing, public health, health promotion, community development or a related field, or a combination of training and experience in an area related to the program, project, or service. International education and experience, and additional training or certifications in health literacy and cultural competence are an asset.

Experience

Two to three years of experience in seniors' care, experience working with seniors and diverse populations from different cultures and socio- economic backgrounds; community building, project planning, development, and coordination. Experience working with cultural communities and newcomer populations. Demonstrated experience in developing and implementing health literacy campaigns, community outreach, and presentations across diverse populations and life stages.

Physical Requirements

Sitting, standing, bending, and ability to lift 10 k.

A current and satisfactory Criminal Record Check

Fluency in an additional language is preferred

Valid driver's license and a reliable vehicle with appropriate insurance will be an asset

Must be able to commute to visit clients in their homes

HOURS: 35 hours for one year contract, possibility of renewal dependent on funding.

The successful candidate may be permitted to work a hybrid work model (combination of in-office and remote work).

Candidate must be able to commute to Boundary Office and Highgate Office.

BENEFITS:
