OVERVIEW: Reporting to the Sacred Care Case Manager, the Sacred Care Shkabeh will be responsible for providing direct in-home support services. The Sacred Care Shkabeh will work within a team that provides comprehensive case management services and supports a holistic circle of care.

KEY RESPONSIBILITES:

  • Supports youth in the development of their daily living skills, based on their needs and goals.
  • Access community resources in the fields of mental health, addictions, and related social services.
  • Collaborate with community resources focused on wellness services and supports.
  • Fostering a safe and supportive environment that empowers youth to explore and respond to their emotional and psychological needs.
  • Complete and submit required service documentation.
  • Utilize a relationship-based approach to deescalating and responding to emergencies as they arise.
  • Maintain timely and effective communication with the holistic circle of care to support the care of youth.
  • Other duties as assigned.

QUALIFICATIONS:

  • Strong understanding of approaches to supporting life skills development.
  • Knowledge of community-based resources.
  • Knowledge in the areas of Addictions, Attachment, Mental Health & Trauma and how it impacts young people.
  • Knowledge of the Child and Family Services (CFS) sector & foster care legislation.
  • Demonstrate knowledge and understanding of First Nations experiences, culture and philosophies.
  • Demonstrated skills and competence to establish supportive relationships.
  • Knowledge of assessment and crisis intervention.
  • Ability to work flexible schedule in order to accommodate the needs of the youth.
  • Satisfactory Child Abuse, Adult Abuse, Vulnerable Person Check and Criminal Record Check.
  • Excellent communication, organization, problem solving, and team skills.
  • Ability to speak Anishinaabe preferred.
