INTRODUCTION:

New Path Youth and Family Services is an accredited children's mental health agency providing a range of compassionate, innovative and accessible services for children, youth and their families throughout Simcoe County. PURPOSE OF POSITION:

The Family Support Worker (FSW) is responsible for providing peer support and system navigation to families of children and youth with mental health concerns.

JOB DUTIES & RESPONSIBILITIES:

  1. Ensure quality client outcomes and fidelity to the Family Services Program.
  2. Ensure that service delivery adheres to professional standards recognized by the related Ministry, Accreditation, Quality Assurance and applicable professional disciplines.
  3. Provide direct peer to peer support and system navigation service to children, youth and their families.
  4. Assist families in developing strategies to communicate and work effectively with service providers (e.g., mental health, education, health, youth justice, child welfare), as well as developing tools, strategies, and resources to build family capacity within the non-clinical role.
  5. Develop, implement, and monitor individual family care plans.
  6. Provide family engagement orientation and support to Family Advisory Committee/network pool members/ other families as appropriate.
  7. Co-facilitate peer support groups.
  8. Provide education groups.
  9. Demonstrate sensitivity and commitment to family-centred service and utilize a strengths-based approach.

QUALIFICATIONS:

  1. Diploma or graduate certificate central to the practice of supporting/engaging children, youth and families.
  2. Minimum of 2 years' lived experience as the primary caregiver of a child/youth who has accessed community-based or other child and youth mental health services.
  3. Understanding of family engagement best practices.
  4. Experience navigating the child and youth mental health system and knowledge of community resources within Simcoe County is preferred.

  3. Collaboratively approaches problems from a solutions and strength based perspective by reviewing the viable alternatives and determining the best course of action.
  4. Recognizes, initiates and adapts to changing conditions and situations in the workplace.
  5. Demonstrates awareness for the backgrounds, values, beliefs and traditions of staff, clients, families and communities we serve.
  6. Commits to ongoing training to build the knowledge, awareness and skills that contribute to a creating a culturally safe environment for staff, clients and families.
  7. Effective use of database and Microsoft Office systems.
  8. Confident, articulate and professional when communicating verbally and in writing.
  9. Strength based and engagement skills.
  10. Case management, assessment, and case coordination skills.
  11. French language is an asset.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  1. Required to work flexibly to meet the needs of families, including evenings.
  2. Eligible to work on-site in office and work from home.
  3. Willingness to travel to client homes to provide services using agency/personal vehicle. Must have a valid driver's license with minimum G2.
  4. Prolonged periods of sitting at a desk and working on a computer.
