Nurse Practitioner, Virtual Care Clinic (2-4)

Temporary, Part time (5hrs to 15hrs/week) (until December 31, 2024, with possibility of extension)

South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support and advocacy to address the social determinants of health. Our services and programs are designed to meet the needs of our community.

We are looking to join an Ontario East region-wide initiative that offers free virtual care to residents with or without a primary care provider and with or without a health card. We know there are over 134000 individuals in Ottawa and surrounding areas who are unattached, need timely access to care to address their urgent medical needs. The clinic is open 7 days/week from 1pm-9pm and nurse practitioners will be offering service virtually.

SEOCHC is seeking highly seasoned Nurse Practitioners to serve clients seeking episodic care in a virtual setting. The NP will initiate clinical pathways and refer clients to additional community-based and specialized services as needed, working collaboratively within virtual care team hub and spoke sites across the Ontario Health East region. The NP should exhibit leadership and advocacy skills to promote and support objectives that are consistent with our Centre and the objectives of this region-wide initiative. We are looking to have nurse practitioners that can offer availability for the following shifts: 1pm-6pm, 2pm-7pm & 4pm-9pm (Monday to Sunday).

RESPONSIBILITIES:

  • Perform virtual clinical assessments and therapeutic interventions to clients experiencing an urgent medical need.
  • Contribute to the continuous development, implementation and evaluation of the program and clinical pathways.
  • Complete timely and accurate medical records of client encounters, documentation and third-party correspondence using electronic medical record.
  • Ensure client care and caseload is managed to achieve the goals of virtual care clinic

MINIMUM QUALIFICATIONS:

  • Baccalaureate in Nursing and Diploma/Master of Nursing (Nurse Practitioner)
  • Registered with the College of Nurses of Ontario – Extended class
  • Minimum five years' experience as a nurse practitioner in a community or primary care setting; virtual care experience considered an asset.
  • Strong clinical skills and diagnostic abilities: Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and health education
  • Ability to effectively assess and manage mental health crises in a professional and culturally sensitive manner
  • Previous experience working with culturally diverse and marginalized populations
  • Strong critical thinking, problem-solving skills, organizational and interpersonal skills with an ability to work effectively within a multidisciplinary team environment
  • Ability to work independently and be self-directed
  • Strong communication, interpersonal and conflict resolution skills
  • Proficiency in the use of computers and various software applications
  • Ability to thrive in a busy and fast-paced environment
  • Fluency in English; French and other languages an asset
  • Proof of Vaccination against Covid-19 required
  • Recent Police Record Check for the Vulnerable Sector required
  • Ability to work flexible hours

Hourly Rate: $72.64
