As a Social Recreation Facilitator, you will:

  • Lead the planning, coordination and ongoing improvement of the in-home recreation program, including outreach to underserved areas.

Provide in-home recreation visits directly to participants when suitable volunteer placements are not available.

  • Conduct initial home and participant assessments, follow-up visits and volunteer-participant matching.
  • Maintain supportive communication with volunteers, participants and families; coordinate volunteer schedules and track participation.
  • Help plan, facilitate and evaluate dementia-inclusive social, physical, cognitive and recreational programs.
  • Create safe, welcoming and person-centred experiences for participants and care partners.
  • Maintain confidential program records and assist with volunteer support, community partnerships and program operations.

  • Build trusting relationships with participants, care partners, volunteers and community partners.
  • Are organized and comfortable balancing home visits, group programming and administrative responsibilities.
  • Bring creativity, sound judgment, flexibility and a collaborative approach to your work.
  • Communicate with warmth, professionalism and respect for each person's dignity and independence.

Education & Experience

  • Post-secondary education in community or therapeutic recreation, social work, gerontology or a related social services field.
  • At least two years of experience working with older adults, preferably including people living with dementia or complex needs.
  • At least two years of experience facilitating diverse groups, preferably in social or therapeutic recreation settings.
  • Experience connecting people to community supports and working with volunteers, students or community partners.

Skills & Competencies

  • Knowledge of Alzheimer's disease and other dementias, particularly early- to middle-stage dementia.
  • A commitment to person-centred service and excellent interpersonal, listening and communication skills.
  • Strong planning, organizational, time management and group facilitation abilities.
  • Cultural sensitivity, sound judgment, professionalism and respect for confidentiality.
  • Knowledge of local community resources and referral pathways.
  • Ability to work independently, lead activities and collaborate with a team.
  • Proficiency with Microsoft Office and client databases.

This Role Requires

  • Travel throughout Peterborough, Kawartha Lakes, Northumberland and Haliburton, with occasional travel outside the region.
  • A valid G Class driver's licence and regular access to a reliable, insured vehicle.
  • A Vulnerable Sector Police Check conducted within the last two months.
  • Valid First Aid/CPR certification is required. Seniors Fitness Instructor Certification (SFIC), therapeutic recreation certification or TRO membership are considered assets.
  • Flexibility to work occasional evenings or weekends; successful completion of Society-provided Minds in Motion® and dementia-specific training.

Working Conditions

  • Regular travel across the four-county service area is required.
  • This is an active, physical role that includes participating in recreational and exercise activities and assisting with demonstrating and modelling exercises for participants, under the direction of the exercise lead. The role also involves managing multiple participant needs, occasionally responding to challenging situations, moving program equipment and supplies weighing up to 25 pounds, and working non-standard hours when needed.
