Job Description

Reporting to the Clinical Services Manager, Inpatient Surgery, Outpatient Department and Fracture & Plastics Clinics, the Patient Navigator (PN) for the Urgent Medical Clinic (UMC) at Etobicoke General Hospital (EGH) will play a critical role in developing and coordinating a multi-disciplinary approach to care, establishing partnership within and external to EGH. As part of ongoing operations, the Patient Navigator will support: triaging and prioritization of referrals; preliminary intake and initial assessment; coordination of clinic flow; collaboration with professional practice to coordinate patient care; coordination of required diagnostics, ensuring follow-up; reviewing and communicating diagnostics results; appropriate follow-up regarding specialty services and / or consults. This is an exciting opportunity for an energetic, highly motivated and organized individual with sound critical thinking skills.

Accountabilities:

  • In consultation with the Clinical Services Manager, Director(s) and Physician Lead(s), develops and implements relevant triage and assessment tools, as well as any other tools / processes required to establish good clinic flow and follow-up.
  • Triaging and prioritization of patient referrals.
  • Completes preliminary intake and initial patient assessments.
  • Collaborates with professional practice to coordinate patient care, as required.
  • Coordination of required diagnostics, ensuring appropriate follow-up.
  • Reviews and communication of diagnostics results, raising alerts when required.
  • Appropriate follow-up regarding specialty services and / or consults within and external to EGH.
  • Collaborates with primary care and other community services to support ongoing wellness and care, as required.
  • Provides high quality care, inclusive of a positive patient / family experience, identifying areas of improvement.
  • Fosters and maintains effective interprofessional and interdepartmental relationships and participates in activities which contribute to the effective operations and flow function of the Urgent Medical Clinic at EGH.
  • Participates in the ongoing development of the Urgent Medical Clinic, which may include collaboration with the services offered at the Brampton Civic and Peel Memorial sites
Qualifications
  • Current registration with The College of Nurses of Ontario as a Registered Nurse
  • BScN required, Maters Degree preferred
  • Current professional affiliation / membership, such as Registered Nurses Association of Ontario (RNAO) an asset
  • Recent experience within the last 5 years, with a minimum 2 years Medical or Surgical experience
  • Experience in an ambulatory setting preferred
  • Must agree to participate in an online Patient Navigation Course (De Sousa Institute) immediately after hire and attend a one-day workshop in Toronto.
  • Demonstrated ability to establish and foster strong and positive patient / partner / staff relationships
  • Strong problem solving and critical thinking skills
  • Effective verbal and written communication skills
  • Highly motivated team player
  • Evidence of collaboration across multiple disciplines / divisions / programs / services
  • Ability to travel between campuses
  • Current BCLS required
  • ACLS preferred
  • A combination of experience and academic qualifications will be considered

Additional Information

Hours: Currently Days, including weekends and statutory holidays (subject to change in accordance with operational requirements)

SALARY:
Minimum: $50.52 per hour
Maximum: $65.01 per hour
