Long-Term Care Licensed Practical Nurse

We are seeking a local to Nanaimo BC LPN interested in working LTC / at Wexford place at this time. Your clinical skill and compassion directly improve quality of life for vulnerable adults while supporting facilities that rely on our rigorously vetted professionals. This role exists because residents deserve consistent, expert nursing that only a dedicated LPN can provide.

• Provide daily personal and clinical care to 8–12 long-term care residents, achieving 95%+ resident and family satisfaction scores within the first 90 days.
• Administer medications and treatments with 100% accuracy, reducing medication errors to zero through precise documentation and double-checks.
• Identify and escalate changes in resident condition within 15 minutes, contributing to a 20% reduction in preventable hospital transfers.
• Collaborate with the interdisciplinary team to update care plans weekly, ensuring every resident's goals are met and documented.
• Mentor one new or junior LPN per quarter, raising unit competency scores by at least 10%.
• Maintain infection-control and safety standards that keep facility audit scores above 95%.

You report to the facility's Charge Nurse or Director of Care and work alongside a team of RNs, PSWs, and therapists. Advanced Care Group's RN Placement Managers provide ongoing support, scheduling flexibility, and access to continuing-education resources. Standard long-term care tools, electronic health records, and a dedicated clinical educator are available on site.

• Current LPN/RPN license in good standing (British Columbia or equivalent jurisdiction).
• Minimum 12 months recent canadian long-term care or geriatric experience with proven medication administration competence.
• Demonstrated ability to manage a caseload of 8+ residents while meeting documentation and safety standards.
• Strong English communication skills for resident, family, and team interactions.
• Willingness to work rotating shifts including weekends and holidays.
