Wage: $45 per hour

Leq'á:mel First Nation (LFN) is located approximately 22 kilometres east of Mission, British Columbia. LFN continues to assert its inherent right of self-government throughout its Territory, which spans a significant portion of the Lower Mainland. Home to approximately 800 Leq'á:mel Xwélmexw (People), the Nation is governed by Chief and Council under its Custom Election Code. Join Our Team

Leq'á:mel First Nation is seeking a compassionate, professional, and community-focused Community & Public Health Registered Nurse (RN) to provide culturally safe, trauma-informed, and client-centred nursing services to individuals, families, and the community.

The RN will provide a range of community and public health services, including health promotion, disease prevention, immunization, health education, nursing assessments, care planning, chronic disease support, home and community care, referrals, and client advocacy.

The RN will provide clinical supervision, mentorship, coaching, and practice support to the Licensed Practical Nurse (LPN) and Home Health Community Care Aide. The RN will coordinate nursing duties based on client needs, the LPN's scope of practice and competence, professional standards, legislation, and LFN policies.

The successful candidate will collaborate with the Nursing Projects Coordinator, community staff, healthcare professionals, First Nations Health Authority partners, Elders, families, and external service providers to promote continuity of care and positive health outcomes.

What You Will Do

Key Responsibilities

• Provide safe, competent, and professional nursing care in accordance with BCCNM standards, applicable legislation, and LFN policies.
• Conduct nursing assessments, develop and evaluate client-centred care plans, provide follow-up, and monitor client outcomes.
• Provide community-based nursing services through clinics, home visits, telephone support, outreach, and other community settings.
• Provide immunizations and support health promotion, disease prevention, chronic disease management, cancer screening, communicable disease prevention, harm reduction, and health education.
• Provide clinical supervision, mentorship, coaching, and practice support to the LPN and Home Health Community Care Aide; assign and delegate duties appropriately based on client needs, care plans, scope of practice, competency, and professional standards.
• Develop and update individualized care plans for clients requiring assistance with activities of daily living, promoting independence, dignity, privacy, choice, cultural safety, infection prevention, and workplace safety.
• Monitor care delivery and documentation, reassess client needs, and address changes in condition, incidents, unsafe practices, or other concerns through appropriate follow-up and escalation.
• Support clients and caregivers in achieving health and wellness goals and accessing appropriate healthcare and community services.
• Build respectful, trusting, and culturally safe relationships with community members, families, Elders, and healthcare partners.
• Coordinate referrals, follow-up, and transitions between community, primary, acute, and specialist healthcare services.
• Maintain accurate, timely, and confidential clinical documentation and protect client health information.
• Participate in community health initiatives, program planning, quality improvement, and other health and wellness activities.
• Maintain professional competency through ongoing education, training, and professional development.
• Perform other related duties as assigned.

Qualifications

• Bachelor's degree in nursing from a recognized university.
• Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) as a Registered Nurse.
• Minimum of two (2) years of recent experience in public health, community health, or home care nursing.
• Valid First Aid and CPR Level C certification.
• Strong clinical assessment, judgment, critical thinking, communication, and organizational skills.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Ability to build respectful, positive relationships with clients, families, community members, and healthcare professionals.
• Proficiency with computers and common office software, including Microsoft Word and Outlook.

Assets

• Experience working in a First Nations community, Indigenous organization, First Nations government, or not-for-profit organization.
• Knowledge of Leq'á:mel culture and history or a demonstrated willingness to learn.
• Experience working in community-based nursing, public health, or home care settings.

Additional Requirements

• Must be able to successfully complete a Criminal Record Check with Vulnerable Sector Screening.
• Valid BC Class 5 driver's licence, reliable transportation, and a clean driver's abstract are required.
• Must maintain confidentiality and comply with professional, organizational, and privacy requirements.
• Occasional travel may be required for training, meetings, home visits, community visits, and other work-related activities.

Working Conditions

• Monday to Friday, 8:00 a.m. – 4:00 p.m. Days to be determined.
• Must be physically capable of performing the duties and responsibilities of the nursing position, including those required for community-based care, as required
• Extended periods of sitting and computer use.
• Attendance at scheduled meetings and occasional travel may be required.
