Community Liaison - Referral & Client Acquisition

THE ROLE

Dibac Home Support is seeking a Community Liaison — Referral & Client Acquisition to represent our agency across the KWC region. This role is focused on building referral pipelines, expanding Dibac's presence, and acquiring new clients through strategic outreach. You will connect with physicians, hospitals, social workers, retirement communities, and community organizations to ensure Dibac is consistently top‐of‐mind when their clients need in‐home support. The Community Liaison is responsible for referral generation, client acquisition, and community outreach—all essential to the agency's growth.

WHAT YOU WILL DO

• Build and maintain referral relationships with physicians, hospital discharge planners, social workers, retirement communities, and community organizations.
• Acquire new clients by introducing Dibac's services to referral partners and guiding interested families toward onboarding.
• Conduct in‐person outreach visits, presentations, and follow‐ups to keep Dibac top‐of‐mind.
• Respond quickly to referral and client inquiries, ensuring smooth handoffs to the coordinating team.
• Attend community events, health fairs, and networking opportunities to expand Dibac's visibility and client pipeline.
• Distribute Dibac marketing materials to referral partners and prospective clients.
• Track outreach activity, leads, and client conversions, reporting regularly to the Director.
• Identify new referral sources and acquisition opportunities across the KWC region.

This role is directly responsible for driving referrals and converting inquiries into active clients.

WHAT WE ARE LOOKING FOR

• Between 3 - 5 years in community outreach, business development, healthcare sales, or another client‐acquisition‐focused role.
• Strong interpersonal and communication skills.
• Self‐directed and able to manage an independent schedule.
• Knowledge of or interest in the home care and community health landscape in Waterloo Region.
• Valid driver's licence and reliable vehicle for travel across KWC.
• Second language an asset (French, Punjabi, Arabic, Mandarin, or others).
• Legally eligible to work in Canada.
