The Opportunity

Are you ready to help young adults flourish like a palm tree and grow like a cedar? At Eden Community Living Services, we are looking for dedicated mentors to join our Transition-Aged Youth (TAY) program. This is a front-line position focused on the direct support and empowerment of young adults with developmental disabilities (ages 18–25).

Key Responsibilities

· Direct Daily Support: Provide hands-on assistance with Activities of Daily Living (ADLs), including support with personal hygiene as needed (showering, grooming, and dressing) while promoting maximum independence and resident dignity.

· The Launchpad Execution: Implement our signature weekly schedule, focusing on transit training, job search support, and independent living skills.

· Empowerment & Advocacy: Work directly with residents to achieve their Individual Support Plan (ISP) goals, acting as a coach and mentor in the community.

· Compliance & Documentation: Maintain meticulous daily logs, medication administration records (MARs), and incident reports in full alignment with QAM (Ontario Regulation 299/10).

Mandatory Certification & Record Check Requirements

To ensure the highest safety standards for our residents and meet provincial regulations, all successful candidates must provide:

· Broad Record and Vulnerable Sector Check (VSC): A clear and original VSC conducted by your local police service (e.g., Peel Regional Police) dated within the last 6 months. Note: A basic Criminal Record Check is not sufficient; a broad search of the vulnerable sector database is a mandatory legal requirement.

· Educational Credentials: DSW, SSW, CYW, or equivalent human services diploma/degree.

· Standard First Aid & CPR Level C: Must be current.

· Crisis Intervention: Valid certification in CPI, Safe Management, or an equivalent approved de-escalation technique.

Position Details

· Role Level: This is a Front-Line Specialist position

· Shift Work: Candidates must be available for a variety of shifts, including mornings, evenings, and weekends.
