Job Title: Client Intervention Program Worker (CIPW)

Reports To: Client Intervention Worker

Location: Momiji Headquarters – 3555 Kingston Rd, Scarborough, ON, M1M 3W4

Salary or Salary Range: $20 - $22 per hour

Job Type: Full-Time, Temporary (Minimum 1-Year Contract)

Job Summary:

The Client Intervention Program Worker (CIPW) plays an important role in supporting seniors and their caregivers by assisting with client intervention services, community resource navigation, and educational programming. Working closely with the Client Intervention Worker (CIW), this position helps clients access the supports they need while promoting dignity, independence, and quality of life. The successful candidate will provide direct client support, maintain accurate documentation, assist with assessments and program delivery, and collaborate with community partners to ensure seniors receive timely, appropriate services. This is an ideal opportunity for a compassionate and organized social services professional who is passionate about improving the lives of older adults and contributing to a supportive, multidisciplinary team.

Working Hours:

Momiji operates seven days per week. This is a full-time position, typically working Monday to Friday, 9:00 AM – 5:00 PM. Flexibility is required for occasional evenings and weekends to support programs, workshops, community events, and operational needs. Work is performed primarily onsite at Momiji's Headquarters, with occasional travel throughout the community to support clients and programs.

Vacancy Status:

This posting is for an existing temporary vacancy with a minimum one-year term.

Key Responsibilities:

  • Support the delivery of client intervention services for seniors and their caregivers.
  • Assist with client assessments, supportive interviews, and intake processes under the guidance of the Client Intervention Worker.
  • Develop, coordinate, and deliver educational workshops, information sessions, and community programs.
  • Support medical appointment coordination and other client service requests.
  • Maintain accurate client files, case documentation, and confidential records.
  • Research, develop, and maintain community resource information for seniors, tenants, and caregivers.
  • Assist with data collection, program statistics, reporting, and quality improvement initiatives.
  • Prepare reports, meeting minutes, and administrative documentation to support departmental operations.
  • Attend community meetings and collaborate with external service providers as assigned.
  • Promote a client-centred approach while ensuring services align with organizational policies, professional standards, and applicable legislation.
  • Support other client intervention, organizational initiatives, and other duties as assigned.

Required Skills, Qualifications & Experience:

  • Social Service Worker (SSW) Diploma, or equivalent combination of education and relevant experience.
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  • Minimum one (1) year of experience working with seniors, caregivers, or within community-based social services.
  • Proficiency in Microsoft Office and experience working with electronic databases.
  • Knowledge of community resources, social services, and relevant legislation affecting seniors.
  • Ability to manage multiple priorities while maintaining professionalism, empathy, and attention to detail.
  • Experience conducting client assessments, supportive interviewing, or case management is an asset.
  • Excellent interpersonal, communication, problem-solving, and organizational skills.
  • Japanese language skills are considered a strong asset.
  • Valid driver's licence and access to a reliable vehicle are considered assets.
