Social Worker – Full-Time (Flexible Schedule)

About the Role

Join a welcoming interdisciplinary team including physicians, nurse practitioners, nursing staff, and another Social Worker
Work collaboratively to support patients with complex and evolving needs
Contribute to thoughtful, coordinated, team-based care

What You'll Do

Provide counselling and psychosocial support across the lifespan
Support patients with:
- Mental health concerns
- Addictions
- Chronic illness
- Life transitions
Balance both brief, solution-focused support and longer-term therapeutic relationships
Collaborate with team members on care planning and shared decision-making
Participate in case discussions and team meetings
Contribute to the development of programs and services that reflect community needs

What We Offer

Full-time position with flexible scheduling
