Caseload Overview

We are currently recruiting for a part-time SpeechLanguage Pathologist to support an adult community caseload funded through Ontario Health at Home. Services are delivered in clients homes, longterm care or retirement homes, and approved community settings.

You will have the opportunity to:

  • Accept referrals that align with your availability, geography, and clinical interests
  • Build a caseload at a pace that works for you
  • Provide services primarily focused on communication and swallowing in adult and olderadult populations

Responsibilities Include

  • Completing comprehensive speechlanguage pathology assessments for adults, including:
    • Communication (speech, language, cognitivecommunication, voice, fluency)
    • Swallowing and feeding (dysphagia)
  • Developing and implementing individualized, clientcentred treatment plans aligned with best practice and Ontario Health at Home service parameters
  • Providing functional, practical intervention strategies that support safety, independence, and participation at home
  • Educating and coaching clients, families, and caregivers to support carryover of strategies into daily routines
  • Monitoring progress, adjusting interventions as needed, and determining readiness for discharge or followup
  • Collaborating with clients, caregivers, and interdisciplinary team members to support coordinated care
  • Maintaining timely, accurate clinical documentation in accordance with 1to1 Rehab and Ontario Health at Home requirements

Qualifications

  • Masters Degree in SpeechLanguage Pathology
  • Registration in good standing with CASLPO
  • Experience working with adult or olderadult populations preferred (but not required)
  • Strong clinical reasoning, assessment, and intervention skills
  • Excellent communication, organization, and interpersonal abilities
  • Professional liability insurance
  • Access to a reliable vehicle and valid drivers license
  • Clear Police Vulnerable Sector Check (within the last year)
